Fixed pruritic spots around the anus?

Dear Doctor,
Over the past year or two, I have noticed a small itchy spot near the skin close to my anus (about 1 cm away from the anal opening).
It is approximately 0.5 cm in size.
When I wipe myself clean or scratch the area, it feels very easy to break the skin (the water from my bidet is extremely painful when it touches that area).
The itchiness is particularly noticeable when I touch that spot while scratching.
Generally, it doesn't itch suddenly; most of the time, I only feel it after wiping (the toilet paper at my workplace is very rough).
I tried to take a picture with my phone, but I couldn't see any abnormalities at the itchy spot, and there are no signs of sweat rash (which I had experienced a long time ago).
There is no sensation of any foreign body when I press on it.
Currently, I occasionally apply cream for sweat rash to that area.
Given that I have consistently experienced itchiness at the same spot over the years, but there are no visible abnormalities on the surface, what could be the possible causes?

Persistent itching near the anus can be a frustrating and uncomfortable issue, and it’s important to understand the potential causes and solutions. Based on your description, there are several possibilities to consider.

Possible Causes
1. Irritation or Allergic Reaction: One of the most common causes of anal itching is irritation from hygiene products, soaps, or even toilet paper. You mentioned that the toilet paper at your workplace is rough, which could contribute to irritation. Additionally, if you use any scented products or wipes, these could also lead to allergic reactions.

2. Skin Conditions: Conditions such as eczema or psoriasis can cause localized itching. Although you mentioned that there are no visible signs, these conditions can sometimes present with minimal symptoms initially.
3. Infections: Fungal infections, such as candidiasis, can occur in the moist areas around the anus. These infections may not always present with visible symptoms but can cause significant itching.
4. Hemorrhoids: Swollen veins in the rectal area can lead to itching and discomfort. Even if you don’t see visible signs, internal hemorrhoids can still cause symptoms.

5. Pinworms: Particularly in children, pinworms can cause intense itching around the anus, especially at night. This is less common in adults but still a possibility.

6. Dietary Factors: Certain foods, such as spicy foods, caffeine, or alcohol, can irritate the anal area and lead to itching.

Solutions and Recommendations
1. Hygiene Practices: Ensure that you are cleaning the area gently but thoroughly after bowel movements. Consider using unscented, soft toilet paper or wet wipes designed for sensitive skin. Avoid harsh soaps or products in the area.

2. Topical Treatments: Since you have been using a cream for sweat rashes, it may help to continue using it if it provides relief. However, be cautious with over-the-counter treatments, as they may not address the underlying issue.

3. Avoid Scratching: While it may be tempting to scratch the area, this can lead to further irritation or even infection. Keeping your nails trimmed and avoiding tight clothing can help reduce the urge to scratch.

4. Consult a Dermatologist: If the itching persists, it would be wise to consult a dermatologist. They can perform a thorough examination and may recommend a skin biopsy or other tests to rule out conditions like eczema or psoriasis.

5. Consider Dietary Changes: If you suspect that certain foods may be contributing to the issue, try keeping a food diary to identify any potential triggers.
6. Medication: If a fungal infection or another condition is diagnosed, your doctor may prescribe antifungal creams or other medications to treat the underlying cause.

7. Regular Check-ups: Since this has been a persistent issue for a couple of years, regular check-ups with your healthcare provider can help monitor the situation and adjust treatment as necessary.

In conclusion, persistent itching near the anus can stem from various causes, including irritation, skin conditions, infections, or dietary factors. It’s essential to maintain good hygiene, avoid irritants, and consult a healthcare professional for a proper diagnosis and treatment plan. By addressing the issue with the right approach, you can find relief and improve your quality of life.
