Sudden onset of a lump?

Hello doctor, yesterday while I was showering, I felt a lump between my left testicle and anus, closer to the testicle.
It is hard and does not move.
I am not sure what could be causing it, but recently I have been experiencing bleeding during bowel movements, and there is also itching around the anus.
I feel some pain when sitting, and it hurts when I touch it.
What could this be?

If there is a hard lump in the perineum, first check if it is painful.
If it is painful, it is more likely related to inflammation, which may include sebaceous gland inflammation, epididymitis, lymphadenitis, anal abscess, or fistula.
If the symptoms are significant and there is a change in size, it is important to seek medical evaluation promptly.
If there is bleeding during bowel movements and pain upon palpation, common conditions in young individuals include anal fissures and hemorrhoids.
These two conditions are not necessarily related, but they can be evaluated together in a colorectal clinic.

More Info

When you discover a lump in the testicular or rectal area, it can understandably cause concern. Based on your description, the lump is located near the left testicle, is hard, non-mobile, and associated with symptoms such as rectal bleeding, itching around the anus, and pain when sitting or touching the area. These symptoms warrant a thorough evaluation by a healthcare professional, as they could indicate a variety of conditions.

Possible Causes
1. Epididymal Cyst or Spermatocele: These are fluid-filled sacs that can develop in the epididymis, which is located at the back of the testicle. They are usually benign and can feel firm. However, they typically do not cause pain or discomfort unless they become large.

2. Inguinal Hernia: This occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. An inguinal hernia can create a lump in the groin or scrotum and may cause pain, especially when straining or lifting.

3. Testicular Tumor: While less common, a hard lump in the testicular area could indicate a testicular tumor, which may be benign or malignant. Testicular cancer often presents as a painless lump, but it can also cause discomfort or pain.

4. Hemorrhoids: Given your symptoms of rectal bleeding and itching, you may also be experiencing hemorrhoids, which are swollen veins in the lower rectum and anus. They can cause discomfort, itching, and bleeding during bowel movements.

5. Anal Fissures or Abscesses: The pain and bleeding during bowel movements could also be due to anal fissures (small tears in the lining of the anus) or abscesses (collections of pus). These conditions can cause significant discomfort and may require treatment.

6. Infection or Inflammation: Conditions such as epididymitis (inflammation of the epididymis) or prostatitis (inflammation of the prostate) can also cause pain and lumps in the genital area.

Recommended Actions
Given the symptoms you've described, it is crucial to seek medical attention promptly. A healthcare provider will likely perform a physical examination and may recommend imaging studies, such as an ultrasound of the scrotum, to evaluate the lump further. Blood tests may also be conducted to check for signs of infection or other underlying conditions.

When to Seek Immediate Care
You should seek immediate medical attention if you experience any of the following:
- Severe pain in the testicular area.

- Sudden swelling of the scrotum.

- High fever or chills.

- Persistent rectal bleeding.

- Any changes in bowel habits or severe abdominal pain.

Conclusion
While some causes of lumps in the testicular or rectal area may be benign, others can be serious and require prompt evaluation and treatment. It is essential not to ignore these symptoms, especially with the presence of rectal bleeding and pain. A healthcare professional will provide the necessary evaluation and guide you through the appropriate treatment options based on the diagnosis. Please make an appointment as soon as possible to address your concerns and ensure your health and well-being.
