Early pregnancy
Hello Dr.
Huang, I have a question.
My wife is eight weeks pregnant.
The ultrasound checks have all been normal with a heartbeat.
However, she often has yellow discharge and occasionally experiences a little bit of bleeding when she is very tired.
The doctor has advised her to take more progesterone (previously one pill, now two pills three times a day) and to rest more.
She is very worried because she has had two previous miscarriages between weeks 4 to 6 of pregnancy.
1.
Dr.
Huang, is this normal?
2.
Are there any adverse effects from taking progesterone?
Thank you!

Dr. Huang Jianxun reply Obstetrics and Gynecology
In early pregnancy, symptoms such as abdominal pain and bleeding may indicate a miscarriage.
It is important to rest appropriately and consider progesterone (also known as luteal support) to help maintain the pregnancy.

Hello! Thank you for your questions regarding your wife's early pregnancy concerns. It's understandable to feel anxious, especially given her history of previous miscarriages. Let's address your questions one by one.
1. Is it normal to have yellow discharge and occasional light bleeding during early pregnancy?
Yellow discharge can be common during pregnancy, particularly in the first trimester. This discharge is often due to hormonal changes and increased blood flow to the pelvic area, which can lead to more vaginal secretions. As long as the discharge is not accompanied by a foul odor, severe itching, or significant pain, it is generally considered normal. However, any bleeding, even if light, can be concerning, especially given your wife's history. Light spotting can occur for various reasons, including implantation bleeding or cervical irritation, but it’s essential to monitor it closely. If the bleeding increases or is accompanied by cramping or severe pain, it would be prudent to contact her healthcare provider immediately.
2. Are there any adverse effects of taking progesterone?
Progesterone, particularly in the form of micronized progesterone (like Utrogestan), is commonly prescribed during early pregnancy, especially for women who have experienced previous miscarriages or have low progesterone levels. Progesterone plays a crucial role in maintaining the uterine lining and supporting early pregnancy. The dosage your wife is taking seems to be in line with what is often prescribed for those at risk of miscarriage.
While progesterone is generally considered safe during pregnancy, some women may experience side effects, including fatigue, dizziness, breast tenderness, and mood swings. However, these side effects are usually mild and manageable. The benefits of progesterone supplementation, particularly in preventing miscarriage in women with a history of pregnancy loss, often outweigh the potential risks.
It's important to note that while progesterone can help stabilize the pregnancy, it does not guarantee that a miscarriage will not occur. Continuous monitoring through ultrasounds and regular check-ups with her healthcare provider is essential to ensure the health of both your wife and the developing fetus.
In summary, while yellow discharge and light spotting can be common in early pregnancy, it is crucial to keep an eye on any changes and maintain open communication with her healthcare provider. The use of progesterone is generally safe and can be beneficial, especially given her previous experiences. Encourage your wife to rest, stay hydrated, and maintain a positive outlook. If there are any concerns or changes in her symptoms, do not hesitate to reach out to her doctor for further guidance.
Wishing you both the best during this exciting yet challenging time!
