Inability to close the eyes?

Hello, Doctor Gao.
I experience a situation about twice a week where I suddenly feel unable to keep my eyes closed when I want to sleep.
When I manage to close them, I feel an urge to open them again.
Sometimes, I also have a persistent urge to cough.
I am currently taking psychiatric medication and have previously consulted with an ophthalmologist regarding whether this could be related to my psychiatric medication, but I was told it is not.
Could you please help me understand what might be causing my condition?

Dezhong, 10~19 year old female. Ask Date: 2022/09/16

Dr. Gao Pilin reply Ophthalmology

Hello, 德中: It should not be related to the medication.
As for what factors are involved, I cannot determine that.
You may consider consulting a neurologist for further evaluation.

Hello! Thank you for your question regarding the difficulty in closing your eyes and the associated symptoms you are experiencing. This situation can be quite distressing, and it's important to explore the potential causes and solutions.

The inability to keep your eyes closed, especially during sleep, can stem from several factors. One of the primary considerations is the role of the muscles that control eyelid movement. The eyelids are primarily controlled by the orbicularis oculi muscle, which is responsible for closing the eyelids. If there is any dysfunction or fatigue in this muscle, it may lead to difficulty in keeping the eyes closed.

Another potential cause could be related to neurological factors. Conditions such as blepharospasm, which is an involuntary contraction of the eyelid muscles, can lead to difficulty in keeping the eyes closed. Additionally, if you are experiencing anxiety or stress, which can be exacerbated by psychiatric medications, it may lead to muscle tension and difficulty relaxing enough to keep your eyes closed.

You mentioned that you are taking psychiatric medications. While your eye doctor indicated that these medications should not be related to your symptoms, it is still possible that they could contribute indirectly. Some medications can cause side effects such as dry eyes or muscle tension, which might make it uncomfortable to keep your eyes closed. It would be beneficial to discuss these symptoms with your prescribing physician, as they may be able to adjust your medication or suggest alternatives that could alleviate the issue.

The sensation of wanting to cough while trying to close your eyes could also be linked to anxiety or a reflex response. Sometimes, when we are anxious or uncomfortable, our bodies can react in unexpected ways, including coughing or other involuntary movements. This could be a psychosomatic response, where psychological factors manifest as physical symptoms.

In terms of solutions, here are a few strategies you might consider:
1. Relaxation Techniques: Engaging in relaxation exercises, such as deep breathing or mindfulness meditation, may help reduce anxiety and muscle tension, making it easier to close your eyes.

2. Eye Care: Ensure that your eyes are well-lubricated, especially if you are experiencing dryness. Using artificial tears can help keep your eyes moist and comfortable.

3. Sleep Hygiene: Establishing a calming bedtime routine can help signal to your body that it is time to relax and sleep. This may include dimming the lights, reducing screen time before bed, and creating a comfortable sleep environment.

4. Consultation with Specialists: If the problem persists, consider seeking a second opinion from a neurologist or a specialist in sleep medicine. They can provide further insights into whether there are any underlying neurological issues contributing to your symptoms.

5. Medication Review: As mentioned earlier, discussing your symptoms with your psychiatrist may lead to adjustments in your medication that could alleviate the side effects you are experiencing.

In conclusion, while the inability to close your eyes can be concerning, understanding the potential causes and exploring various solutions can help you manage the situation more effectively. It is crucial to maintain open communication with your healthcare providers to ensure that all aspects of your health are being considered. If you have any further questions or concerns, please feel free to reach out.
