Fever of unknown origin (FUO)

Since being diagnosed on May 25, my body temperature has been consistently elevated, around 38.4°C.
However, since August, my temperature has generally risen to around 38°C.
I underwent a series of tests at the Veterans General Hospital, and everything came back normal.
The infectious disease specialist suggested that I consult a hematologist to continue searching for the cause.
I would like to inquire about possible diseases associated with fever, as my physical strength fluctuates with the fever, and I am unable to attend classes.
I want to understand what other potential diseases could be involved.

Dr. Liao Yaopan reply Family Medicine

Hello, the causes of unexplained fever may be related to various systems, including infections, autoimmune diseases, and tumors, among others.
Infections are likely the most common cause.
Hematology and rheumatology should also be investigated.

Unexplained fever can be a perplexing and distressing symptom, especially when it persists over an extended period. In your case, having a sustained elevated temperature of around 38°C (100.4°F) since your COVID-19 diagnosis on May 25 raises several concerns that warrant further investigation. While you have undergone a series of tests at a medical facility, and the results have returned normal, it is essential to consider a broad range of potential causes for your ongoing fever.

Potential Causes of Unexplained Fever
1. Infectious Diseases:
- Viral Infections: After a COVID-19 infection, some individuals may experience prolonged symptoms, including fever, due to post-viral syndromes. Other viral infections, such as Epstein-Barr virus (EBV) or cytomegalovirus (CMV), can also lead to prolonged fevers.

- Bacterial Infections: Conditions like endocarditis, osteomyelitis, or abscesses can cause persistent fevers. These infections may not always present with obvious symptoms, making them harder to diagnose.

- Tuberculosis (TB): This chronic infection can manifest with prolonged fever, night sweats, and weight loss. It’s essential to rule out TB, especially if there are risk factors involved.

2. Autoimmune Disorders:
- Conditions such as Still's disease, lupus, or rheumatoid arthritis can present with fevers as part of their symptomatology. These diseases often involve systemic inflammation, which can lead to fluctuating fevers and other systemic symptoms.

3. Malignancies:
- Certain cancers, particularly lymphomas and leukemias, can present with unexplained fevers. These fevers may be accompanied by other symptoms such as night sweats, weight loss, or fatigue.

4. Endocrine Disorders:
- Conditions such as hyperthyroidism can lead to increased body temperature and metabolic rate, resulting in persistent fever.

5. Medications and Drug Reactions:
- Some medications can induce fever as a side effect or due to an allergic reaction. If you have started any new medications recently, this could be a consideration.

6. Chronic Inflammatory Conditions:
- Conditions like sarcoidosis or inflammatory bowel disease can also lead to persistent fevers.

Next Steps
Given that your initial tests have returned normal, it is prudent to follow the recommendation of your infectious disease specialist and consult with a hematologist. They may consider additional tests, including:
- Blood Cultures: To identify any hidden infections.

- Autoimmune Panels: To check for markers of autoimmune diseases.

- Imaging Studies: Such as CT scans or MRIs to look for hidden abscesses or tumors.

- Bone Marrow Biopsy: If there is a suspicion of hematological malignancy.

Managing Symptoms
While you pursue a diagnosis, managing your symptoms is crucial. Here are some strategies:
- Hydration: Ensure you are well-hydrated, as fever can lead to dehydration.

- Rest: Prioritize rest to help your body recover and manage fatigue.

- Antipyretics: Medications like acetaminophen or ibuprofen can help manage fever and discomfort.

Conclusion
Unexplained fever can stem from a variety of causes, and it is essential to continue working with your healthcare providers to identify the underlying issue. Keep a detailed record of your symptoms, including any changes in your fever pattern, associated symptoms, and any new medications or lifestyle changes. This information can be invaluable in guiding your healthcare team toward a diagnosis and appropriate treatment. Remember, persistent fever is a signal from your body that something may be amiss, and it deserves thorough investigation.
