Sudden interruption of breathing?

Hello Doctor: I am a 32-year-old female.
For the past two days, I have been experiencing a sudden sensation of breathlessness (or a feeling of my heart skipping a beat, which is hard to describe...).
Each episode lasts about half a second, and sometimes I have 5 or 6 episodes within an hour, while at other times, several hours can pass without any episodes.
I do not notice any particular symptoms during exercise.
This situation usually occurs more frequently in the afternoon and evening.
What could be the cause? If I need to seek medical attention, which specialty should I consult?

Cynthia, 30~39 year old female. Ask Date: 2022/10/04

Dr. Wang Lanfeng reply Pulmonology

Hello: Based on your description, your symptoms seem more like paroxysmal arrhythmia, which typically occurs during periods of fatigue or increased stress.
I recommend that you visit a cardiology clinic for further evaluation.
Thank you.

Sudden breathlessness can be a concerning symptom, and understanding its potential causes is crucial for determining the appropriate course of action. Based on your description, it seems you are experiencing episodes of breathlessness that may be accompanied by sensations akin to a skipped heartbeat. This can be distressing, especially when it occurs intermittently throughout the day, particularly in the afternoon and evening.

Potential Causes of Sudden Breathlessness
1. Anxiety and Panic Attacks: One of the most common causes of sudden breathlessness is anxiety or panic attacks. During these episodes, individuals may experience hyperventilation, which can lead to feelings of breathlessness, chest tightness, and even palpitations. The body's fight-or-flight response can trigger these symptoms, especially in stressful situations or when one is feeling overwhelmed.

2. Respiratory Issues: Conditions such as asthma or chronic obstructive pulmonary disease (COPD) can lead to sudden breathlessness. Given your history of mild asthma, it is essential to consider whether your symptoms may be related to this condition, especially if you notice any wheezing or coughing.

3. Cardiac Concerns: While less common, heart-related issues can also manifest as sudden breathlessness. Conditions such as arrhythmias (irregular heartbeats) can cause sensations of skipped beats and may be accompanied by breathlessness. It is essential to rule out any cardiac issues, especially if you have risk factors such as a family history of heart disease.

4. Gastroesophageal Reflux Disease (GERD): Sometimes, acid reflux can cause a sensation of breathlessness, particularly if stomach acid irritates the esophagus or if it leads to aspiration into the lungs.

5. Other Medical Conditions: Various other conditions, including anemia, thyroid disorders, or pulmonary embolism, can also lead to sudden breathlessness.

When to Seek Help
Given the intermittent nature of your symptoms and their potential link to anxiety or other medical conditions, it is advisable to seek medical attention. Here are some guidelines on when to see a doctor:
- Frequency and Duration: If your episodes of breathlessness become more frequent or last longer than a few minutes, it is essential to consult a healthcare professional.

- Associated Symptoms: If you experience additional symptoms such as chest pain, dizziness, fainting, or swelling in the legs, seek immediate medical attention.

- Impact on Daily Life: If your symptoms are affecting your ability to perform daily activities or causing significant distress, it is time to seek help.

Which Specialist to Consult
For your symptoms, it would be prudent to start with a visit to a primary care physician. They can conduct an initial evaluation, including a physical examination and possibly some basic tests like an electrocardiogram (ECG) or pulmonary function tests. Depending on their findings, they may refer you to a specialist:
- Pulmonologist: If respiratory issues are suspected, a pulmonologist specializes in lung conditions and can provide further evaluation and treatment.

- Cardiologist: If there are concerns about your heart, a cardiologist can assess your heart health and determine if any interventions are necessary.

- Psychiatrist or Psychologist: If anxiety or panic attacks are suspected to be the underlying cause, a mental health professional can provide therapy and possibly medication to help manage your symptoms.

Conclusion
In summary, sudden breathlessness can arise from various causes, including anxiety, respiratory issues, and cardiac concerns. It is essential to monitor your symptoms closely and seek medical advice, especially if they worsen or significantly impact your quality of life. Starting with a primary care physician is a good first step, and they can guide you to the appropriate specialist based on your symptoms and medical history. Remember, early intervention can lead to better management of your symptoms and improved overall well-being.
