Unexplained fever?
Hello Doctor: Recently, I have been experiencing sudden increases in body temperature, around 37-37.4 degrees Celsius when measured in the ear.
Is this considered a fever? It is also accompanied by fatigue and headaches, but this occurs while I am indoors and at rest.
I usually feel cold, and my average body temperature is in the mid-36s.
Is this normal? I am very concerned that there might be an underlying illness or even cancer that I am unaware of.
Which specialist should I see, and what tests should I undergo? Thank you.

Unexplained fever can be a concerning symptom, and it is understandable that you are worried about your health, especially given your experiences with fluctuating body temperatures and accompanying symptoms like fatigue and headaches. In general, a body temperature of 37.5°C (99.5°F) or higher is considered a fever, especially if it is above your normal baseline temperature, which you mentioned is around 36°C (96.8°F).
Fever is a complex physiological response that can indicate various underlying conditions, ranging from benign to serious. Common causes of unexplained fever include infections (viral, bacterial, or fungal), autoimmune diseases, malignancies (cancers), and even certain medications or environmental factors. Given your symptoms, it is essential to consider the context in which these fevers occur. For instance, if you are experiencing fever without any clear signs of infection (like cough, sore throat, or gastrointestinal symptoms), it may warrant further investigation.
The accompanying symptoms of fatigue and headaches can also provide clues. Fatigue is a common symptom in many conditions, including chronic infections, autoimmune disorders, and malignancies. Headaches can be associated with fever but can also indicate other issues, such as tension headaches or migraines, especially if they are persistent and severe.
Given your concerns about serious conditions like cancer, it is crucial to seek medical advice. You should consider visiting a healthcare provider who can perform a thorough evaluation. This typically includes a detailed medical history, a physical examination, and possibly some laboratory tests. Blood tests can help assess for signs of infection, inflammation, or other abnormalities. Depending on the initial findings, your doctor may recommend imaging studies or referrals to specialists, such as an infectious disease expert or a hematologist, if there are concerns about blood disorders or malignancies.
In terms of what to expect during your visit, your doctor may order tests such as:
1. Complete Blood Count (CBC): This test can help identify signs of infection or anemia.
2. Blood Cultures: If an infection is suspected, blood cultures can help identify the causative organism.
3. Inflammatory Markers: Tests like C-reactive protein (CRP) or erythrocyte sedimentation rate (ESR) can indicate inflammation in the body.
4. Imaging Studies: Depending on your symptoms, X-rays, CT scans, or MRIs may be necessary to look for underlying issues.
If your fever persists or worsens, or if you develop new symptoms (such as significant weight loss, night sweats, or persistent pain), it is crucial to seek immediate medical attention. These could be signs of a more serious condition that requires prompt intervention.
In summary, while occasional mild fevers can be normal, persistent or unexplained fevers, especially when accompanied by other concerning symptoms, should be evaluated by a healthcare professional. Early diagnosis and treatment are key to managing any underlying conditions effectively. Don't hesitate to reach out to a primary care physician or a specialist to discuss your symptoms and concerns further. Your health and peace of mind are important, and getting the right care can help you feel more secure about your well-being.
