Sudden throat itch and cough?
Hello Doctor, I work as a contracted staff member in a hospital, which carries a certain risk of viral infections.
I am a patient with a chronic illness (Type 2 diabetes) and have been on medication with regular follow-ups.
So far, I have not been diagnosed with COVID-19.
I just received my fourth vaccine dose on October 7th (I hope I'm not that unlucky...).
Yesterday, while at work, I suddenly felt a strong urge to cough, and I have been coughing since then.
I feel like I have more phlegm, and my voice is noticeably hoarse.
I looked up COVID-19 symptoms online, and aside from coughing (dry cough), nasal congestion, runny nose, and a low-grade fever, I don't have any other symptoms.
I haven't caught a cold in over a year due to wearing a mask for long periods.
When I cough, my throat hurts slightly.
I have tested negative on a rapid antigen test.
Additionally, I have a breathing issue where I often need to take a deep breath after a while to feel comfortable (this happens frequently).
I also have left chest pain (below the collarbone and above the breast) that comes in waves (this just started recently, with varying intervals—sometimes every few minutes, sometimes every few hours).
I'm very concerned that there might be a problem.
Thank you, Doctor.

Hello: If you experience respiratory symptoms but test negative for COVID-19 with a rapid test, it is recommended that you seek medical attention if the symptoms persist.
A physician can determine whether a COVID-19 PCR test is necessary.
Regarding the chest pain issue you mentioned, since it is an atypical symptom, it is advisable to first visit a thoracic clinic if the symptoms continue.
Thank you.

More Info
Based on your description, it sounds like you're experiencing a combination of symptoms that could be indicative of several underlying conditions. The sudden onset of throat itchiness and persistent coughing, along with increased phlegm production and hoarseness, can be concerning, especially given your background as a healthcare worker and your chronic condition of type 2 diabetes.
Possible Causes of Your Symptoms
1. Viral Infection: Given your work environment, you may have been exposed to various viruses, including those that cause upper respiratory infections. Symptoms like a dry cough, throat irritation, and increased phlegm can often be attributed to viral infections such as the common cold or even COVID-19, despite your negative rapid test. It's important to note that not all infections present with the same symptoms, and some may not be detectable immediately.
2. Allergic Reactions: If you have a history of allergies, sudden throat itchiness and coughing could be a response to allergens in your environment. This could include dust, pollen, or even irritants from wearing a mask for extended periods. Allergic rhinitis can lead to post-nasal drip, which can irritate the throat and cause coughing.
3. Gastroesophageal Reflux Disease (GERD): Your description of needing to take deep breaths frequently could suggest that acid reflux is irritating your throat. GERD can cause throat discomfort and coughing, especially when lying down or after eating.
4. Asthma or Reactive Airways: Given your symptoms of wheezing and the need to take deep breaths, it’s possible that you may be experiencing reactive airway disease or asthma. This is particularly relevant if you have a history of respiratory issues or if you notice that your symptoms worsen with certain triggers, such as cold air or strong odors.
5. Infection or Inflammation: The left chest pain you mentioned, especially if it is new and intermittent, could be related to respiratory issues or even musculoskeletal pain. However, it’s essential to rule out any cardiac concerns, particularly given your diabetes.
Recommendations for Management
1. Consult a Healthcare Provider: Given the complexity of your symptoms and your medical history, it’s crucial to consult a healthcare provider. They can perform a thorough evaluation, including a physical examination and possibly imaging studies or pulmonary function tests, to determine the underlying cause of your symptoms.
2. Symptomatic Relief: In the meantime, consider using over-the-counter antihistamines if allergies are suspected, or throat lozenges to soothe your throat. Staying hydrated can also help thin mucus and ease throat irritation.
3. Monitor Symptoms: Keep a close eye on your symptoms. If you develop a fever, experience worsening chest pain, or have difficulty breathing, seek medical attention immediately.
4. Lifestyle Modifications: If GERD is a concern, consider dietary changes, such as avoiding spicy or acidic foods, and elevating the head of your bed to reduce nighttime symptoms.
5. Stress Management: Since you mentioned feeling anxious about your symptoms, practicing stress-reduction techniques such as deep breathing exercises, meditation, or gentle yoga may help alleviate some of the tension and improve your overall well-being.
Conclusion
Your symptoms warrant careful evaluation, especially considering your chronic health condition. While many of the potential causes are manageable, it’s essential to address them promptly to prevent complications. Please prioritize seeing a healthcare professional who can provide personalized advice and treatment based on a comprehensive assessment of your health.
