Gynecological issues

Hello Doctor, I have several questions and I appreciate your help in clarifying them.
1.
I usually experience breast tenderness before my period, and it typically subsides around the third or fourth day of menstruation.
However, this month, I have been experiencing breast tenderness for almost two weeks.
Is this normal? Additionally, I have been experiencing frequent urination and some bloating in my lower abdomen.
I had sexual intercourse a little over a month ago.
2.
When I wipe after urinating, I am very gentle, but I often experience tearing near the vaginal area close to the anus, which is quite bothersome.
I also find it easy to sustain tearing during intercourse.
Could you please explain why this is happening?
3.
This month, in October, I have been experiencing recurring itching and mild pain.
Occasionally, I feel sharp pain on the left side of my lower abdomen.
I have taken a pregnancy test, which was negative.
Thank you for your assistance, Doctor.

Dear patient,
Thank you for reaching out with your concerns regarding common gynecological issues. I understand that you have several questions, and I will address each one based on the information you've provided, as well as relevant medical knowledge.

1. Breast Tenderness and Frequent Urination: It is common for women to experience breast tenderness (mastalgia) in the luteal phase of their menstrual cycle, which occurs after ovulation and before menstruation. This is typically due to hormonal fluctuations, particularly increases in progesterone. However, if you are experiencing breast tenderness for an extended period (like half a month), it may be worth investigating further. Prolonged breast tenderness could be related to hormonal imbalances, stress, or even conditions like fibrocystic breast changes. Additionally, frequent urination and abdominal bloating can be symptoms of premenstrual syndrome (PMS) or other underlying conditions. If these symptoms persist or worsen, I recommend consulting with a healthcare provider for a thorough evaluation, including hormonal assessments.

2. Tearing During Intercourse and Hygiene: Experiencing tearing or discomfort during intercourse can be distressing. This can occur for several reasons, including insufficient lubrication, vaginal dryness, or anatomical factors. It's important to ensure adequate foreplay and consider using a water-based lubricant to reduce friction. If you are experiencing tearing during wiping after urination, it could be due to sensitive skin or improper wiping technique. Always wipe from front to back to minimize the risk of irritation or infection. If these issues continue, it may be beneficial to consult a gynecologist who can assess for any underlying conditions such as vulvodynia or other dermatological concerns.

3. Itching, Pain, and Abdominal Discomfort: The symptoms of itching and mild pain you are experiencing, along with occasional left-sided abdominal pain, could indicate a few possibilities. Common causes include yeast infections, bacterial vaginosis, or sexually transmitted infections (STIs). It is also possible that the abdominal pain is related to ovulation or other gynecological conditions such as ovarian cysts or endometriosis. Since you have already ruled out pregnancy, I recommend scheduling an appointment with a gynecologist for a pelvic examination and possibly some laboratory tests to identify the cause of your symptoms.
In summary, while some of your symptoms may be common and related to hormonal changes or anatomical factors, persistent or severe symptoms warrant further investigation. It is essential to maintain open communication with your healthcare provider and not hesitate to seek help when needed. Regular gynecological check-ups can help monitor your reproductive health and address any concerns early on.

If you have any further questions or need clarification on any points, please feel free to ask. Your health and well-being are important, and I encourage you to take proactive steps in managing your symptoms.

Best regards.
