Changes in Nipple and Areola: Causes and When to Seek Help - Breast and Thyroid

Nipple and areola?

The skin of my areola seems a bit wrinkled and thinner, and my nipple appears to be inverted.
What could be the reason for this? When I lift my breast, the nipple does not appear inverted, and it also becomes erect when I pull on it.
I have breastfed before; could this be due to sagging? Should I see a doctor for an examination?

Cherry, 30~39 year old female. Ask Date: 2022/10/21

Dr. Yuan Tianmin reply Breast and Thyroid

At 38 years old, you shouldn't be too concerned about breast sagging based on your description; it actually sounds quite normal.
If you're still feeling uneasy, it would be a good idea to get a breast ultrasound for peace of mind.

Changes in the appearance of the nipple and areola can be concerning, especially if they are accompanied by other symptoms. The wrinkling or thinning of the areola, along with the retraction of the nipple, can have several potential causes. Understanding these changes is crucial for determining whether medical evaluation is necessary.

One common reason for changes in the nipple and areola is hormonal fluctuations. Hormones play a significant role in breast tissue changes, especially during different life stages such as puberty, menstruation, pregnancy, and menopause. After breastfeeding, some women may notice changes in the breast tissue, including a decrease in volume or elasticity, which can lead to a more wrinkled appearance of the areola and a retracted nipple. This is often a normal physiological change, but it can be distressing.

Another potential cause could be related to skin conditions. Conditions such as eczema or dermatitis can affect the skin of the areola, leading to changes in texture and appearance. Additionally, infections or inflammatory conditions, such as mastitis, can cause changes in the nipple and surrounding area. If there is any redness, swelling, or pain associated with these changes, it is essential to seek medical attention.

In some cases, changes in the nipple and areola can indicate more serious conditions, such as Paget's disease of the nipple, which is a rare form of breast cancer that typically presents with changes in the nipple and areola. Symptoms may include itching, burning, or a discharge from the nipple, along with changes in color or texture. If you notice any of these symptoms, it is crucial to consult a healthcare professional promptly.

Given your history of breastfeeding and the changes you are observing, it is likely that these changes are related to the natural aging process of breast tissue or hormonal changes. However, it is always best to err on the side of caution. If you are experiencing significant changes, discomfort, or if you have any concerns about the appearance of your nipple and areola, it is advisable to schedule an appointment with a healthcare provider. They can perform a thorough examination, discuss your medical history, and determine if any further testing or intervention is necessary.

In summary, while changes in the nipple and areola can often be benign and related to hormonal changes or the effects of breastfeeding, it is essential to monitor these changes closely. If you have any concerns or if the changes are accompanied by other symptoms, seeking medical advice is the best course of action. Regular check-ups and open communication with your healthcare provider can help ensure your breast health is maintained.
