Bowel-related issues?

Hello doctor, a few days ago I noticed bright red blood at the end of my stool, and I have pain in my anus.
The stool is quite thick, but the latter part breaks apart when flushed.
Over the past two days, I’m not sure if it’s just in my head, but my stool has changed to about the thickness of a five-dollar coin and is a yellow-brown color.
My diet and weight have been normal.
Am I sick?

Wen, 20~29 year old female. Ask Date: 2022/10/25

It appears to be mild anal fissure bleeding, which can be monitored further.

Seeing blood in your stool can be alarming, and it's important to understand the potential causes and when to seek medical attention. The presence of bright red blood at the end of your stool, along with pain during bowel movements, may indicate several conditions, some of which require prompt medical evaluation.

Possible Causes of Blood in Stool
1. Hemorrhoids: One of the most common causes of bright red blood in the stool is hemorrhoids, which are swollen veins in the lower rectum or anus. They can cause pain, itching, and bleeding during bowel movements, especially if the stool is hard or large.

2. Anal Fissures: These are small tears in the lining of the anus, often caused by passing hard or large stools. They can lead to sharp pain and bleeding during bowel movements.

3. Diverticular Disease: Diverticulosis, which involves small pouches forming in the colon wall, can lead to diverticulitis if these pouches become inflamed or infected. This condition can cause bleeding and abdominal pain.

4. Colorectal Polyps or Cancer: While less common, polyps (growths on the colon lining) or colorectal cancer can also cause blood in the stool. This is more likely if there are other symptoms such as weight loss, changes in bowel habits, or abdominal discomfort.

5. Gastrointestinal Infections: Certain infections can cause inflammation and bleeding in the gastrointestinal tract, leading to blood in the stool.

When to Seek Medical Attention
You should seek medical attention if you experience any of the following:
- Persistent Bleeding: If the bleeding continues or worsens, it’s important to consult a healthcare provider.

- Severe Pain: If the pain during bowel movements is severe or persistent, it warrants evaluation.

- Changes in Bowel Habits: Any significant changes in your bowel habits, such as diarrhea or constipation lasting more than a few days, should be discussed with a doctor.

- Accompanying Symptoms: If you experience other symptoms such as fever, chills, unexplained weight loss, or fatigue, you should seek medical care.

- Large Amounts of Blood: If you notice a significant amount of blood or if the blood is dark or tarry, this could indicate a more serious condition.

Recommendations
Given your description of the symptoms, including the presence of bright red blood and pain during bowel movements, it would be wise to consult a healthcare professional. They may perform a physical examination, possibly including a rectal exam, and may recommend further tests such as a colonoscopy to determine the cause of the bleeding.

In the meantime, consider the following:
- Dietary Adjustments: Ensure you are consuming enough fiber to help soften your stool and prevent straining during bowel movements. Foods rich in fiber include fruits, vegetables, whole grains, and legumes.

- Hydration: Drink plenty of fluids to help maintain soft stools.

- Avoid Straining: Try to avoid straining during bowel movements, as this can exacerbate hemorrhoids or fissures.

Conclusion
While the presence of blood in your stool can be due to benign conditions like hemorrhoids or anal fissures, it is essential to take it seriously. Monitoring your symptoms and seeking medical advice when necessary can help ensure that any potential issues are addressed promptly. Remember, it’s always better to err on the side of caution when it comes to your health.
