Testicular issues

Hello Doctor, I have a question that has been troubling me.
I have health anxiety, so I tend to be sensitive to changes in my body.
Recently, while taking a shower, I noticed an oval-shaped hard lump below my right testicle (there seems to be a smaller one on the left side as well).
Out of curiosity, I researched online and found that it could be a sign of testicular cancer, which has made me very anxious.
I live in a rural area and there are no urology clinics nearby, but there is a family medicine clinic.
I visited a family medicine doctor who performed a physical examination and said that the size was normal and he did not feel any hard lump.
However, he mentioned that I should monitor it if it grows.
About two weeks later, I noticed that the lump is more noticeable after showering or after ejaculation, but by the next morning, I can no longer feel it.
Because of my anxiety, I went to a second family medicine clinic (with a different doctor) and after a physical examination, the doctor also did not feel any hard lump.
The family medicine doctor told me that if it were testicular cancer, I would be able to feel it at any time, not just at night.
Despite this reassurance, I am still very anxious and would like to ask if I am being overly suspicious.
Should I consider getting an ultrasound at the hospital? Thank you, Doctor!

It's completely understandable to feel anxious about changes in your body, especially when you have a history of health anxiety. The concern you have regarding the lump you felt in your testicle is valid, and it's good that you've sought medical advice already. Let's break down your situation and address your concerns.

First, it's important to recognize that testicular lumps can arise from various causes, and not all of them are serious. Common benign conditions include epididymal cysts, hydroceles, or varicoceles, which can sometimes feel like lumps. Testicular cancer, while serious, is relatively rare, especially in younger men. The fact that two different doctors performed physical examinations and did not find any lumps is reassuring. They are trained to identify abnormalities, and their assessments suggest that what you are feeling may not be a cause for concern.

Your observation that the lump is more noticeable after certain activities, like showering or ejaculation, could be related to changes in blood flow or the position of the testicles. The scrotum can also change in size and shape due to temperature and other factors, which might explain why you can feel the lump at certain times and not others.
However, your anxiety about the possibility of testicular cancer is understandable. It's crucial to listen to your body and seek further evaluation if you feel something is off. If you continue to feel anxious or if the lump changes in size or consistency, it would be reasonable to request an ultrasound. An ultrasound is a non-invasive imaging test that can provide a clearer picture of what’s happening inside the scrotum and can help rule out any serious conditions.

In terms of your anxiety, it may be beneficial to explore strategies to manage your health-related worries. Cognitive-behavioral therapy (CBT) is an effective treatment for health anxiety and can help you develop coping strategies. Mindfulness and relaxation techniques can also be helpful in reducing anxiety levels.

It's also worth noting that if you find yourself frequently checking for lumps or feeling compelled to seek reassurance from multiple doctors, this could be a sign of health anxiety. Addressing this underlying anxiety can be just as important as addressing any physical symptoms.

In summary, while your concerns are valid, the assessments from the doctors you visited are reassuring. If the lump persists or you continue to feel anxious, consider discussing the option of an ultrasound with your healthcare provider. Additionally, seeking support for your anxiety can help you manage your worries more effectively. Remember, it's always better to err on the side of caution when it comes to your health, but it's also important to take care of your mental well-being.
