Blood after bowel movement?
During high school, I experienced bright red blood on my stool, thinking it was my menstrual period and not paying much attention to it; I only remember that the stool was loose.
In my freshman year of college, I also noticed bright red blood on my stool, which was also loose, and I recall having abdominal pain before passing the stool with blood.
Around January of this year, I again noticed bright red blood after a bowel movement, accompanied by mucus, and the stool was loose.
Recently, I'm not sure if it's psychological or what, but my stomach keeps gurgling after meals, and I have been passing gas frequently.
A few days ago, my stool was loose again but without blood.
My grandfather had rectal cancer, and my father has polyps and hemorrhoids.
I am very worried that I might have colorectal cancer or hemorrhoids.
Do I need to undergo further examinations? Thank you.

Dr. Zhang Shuhao reply Surgery
You can first visit a colorectal clinic for an examination to check for common causes of bleeding, such as hemorrhoids, to assess whether a colonoscopy is necessary.
Unless your grandfather has a young-onset colorectal cancer, the likelihood of developing cancer at the age of 20 is very low, even with a family history of cancer within the third degree of kinship.

Rectal bleeding can be a concerning symptom, especially given your family history of colorectal issues. It's important to understand the potential causes and when to seek medical attention.
First, let's clarify what rectal bleeding can indicate. Fresh red blood on the toilet paper or in the stool often suggests that the bleeding is coming from the lower gastrointestinal tract, such as the rectum or anus. Common causes of rectal bleeding include hemorrhoids, anal fissures, diverticular disease, and inflammatory bowel disease (IBD), among others. However, it can also be a sign of more serious conditions, including colorectal polyps or cancer, especially in individuals with a family history of these diseases.
In your case, the fact that you have experienced rectal bleeding multiple times, particularly with accompanying symptoms like abdominal pain and changes in bowel habits (such as diarrhea), raises the need for further evaluation. The presence of mucus and the history of fresh blood are particularly concerning. Given your family history of colorectal cancer (your grandfather) and polyps (your father), it is prudent to take these symptoms seriously.
Here are some guidelines on when to seek medical attention:
1. Persistent Symptoms: If you continue to have rectal bleeding, especially if it occurs frequently or is associated with other symptoms like abdominal pain, changes in bowel habits, or weight loss, you should see a healthcare provider.
2. Severity of Bleeding: If you experience significant amounts of blood, or if the blood is dark or mixed with stool, this warrants immediate medical attention.
3. Family History: Given your family history of colorectal cancer and polyps, it is advisable to consult a healthcare provider sooner rather than later. Family history can increase your risk for these conditions, and early detection is crucial.
4. Other Symptoms: If you experience symptoms such as severe abdominal pain, fever, or signs of anemia (like fatigue, weakness, or shortness of breath), you should seek medical care immediately.
5. Routine Screening: Depending on your age and risk factors, your doctor may recommend routine screening for colorectal cancer, such as a colonoscopy, especially since you have a family history of colorectal issues.
In summary, while conditions like hemorrhoids and anal fissures are common and often benign, your symptoms and family history suggest that a thorough evaluation is warranted. I recommend scheduling an appointment with a gastroenterologist or your primary care physician to discuss your symptoms in detail. They may suggest diagnostic tests such as a colonoscopy to rule out any serious conditions and to provide peace of mind. Early intervention can significantly improve outcomes, so don't hesitate to seek help.
