Implantation bleeding or menstruation?
Hello Doctor: My period was supposed to start on the 13th, but it began today on the 9th.
My last period was from September 16 to September 21.
On the 21st, I had sexual intercourse, but it was only external friction without penetration.
I took ovulation tests the day before and three days after, and they were all negative.
I took a pregnancy test on November 3 (the 13th day) and it was negative, but this morning (the 9th) I had a little bit of bleeding.
Could this be implantation bleeding or my period?
Sss, 10~19 year old female. Ask Date: 2022/11/09
Dr. Lin Lixuan reply Obstetrics and Gynecology
If there was sexual intercourse on September 21 and pregnancy occurred, it should be detectable by October.
Therefore, the current bleeding is likely normal menstrual bleeding.
If there are any concerns, please consult a nearby obstetrics and gynecology clinic for examination and advice.

More Info
Implantation bleeding and menstruation can often be confused, especially when the timing and characteristics of the bleeding are similar. Understanding the differences between the two can help clarify your situation.
Implantation Bleeding:
Implantation bleeding occurs when a fertilized egg attaches itself to the lining of the uterus, which can happen about 6 to 12 days after conception. This type of bleeding is usually light and may be accompanied by some mild cramping. The characteristics of implantation bleeding include:
1. Color: It is often light pink or brown, rather than the bright red typically associated with menstrual bleeding.
2. Amount: The bleeding is usually very light, often described as spotting, and does not require a pad or tampon.
3. Duration: Implantation bleeding typically lasts a few hours to a couple of days, much shorter than a regular menstrual period.
4. Timing: It usually occurs around the time your period is due, but it can be a few days earlier or later.
Menstruation:
Menstrual bleeding is a part of the menstrual cycle and occurs when the uterine lining sheds due to the absence of pregnancy. The characteristics of menstrual bleeding include:
1. Color: Menstrual blood can vary in color from bright red to dark brown, especially towards the end of the period.
2. Amount: Menstrual bleeding is usually heavier than implantation bleeding and can require the use of pads or tampons.
3. Duration: A typical menstrual period lasts anywhere from 3 to 7 days.
4. Symptoms: Menstruation may be accompanied by symptoms such as bloating, breast tenderness, and cramps.
In your case, since you mentioned that your expected period is on the 13th and you experienced light bleeding on the 9th, it could potentially be implantation bleeding, especially if the bleeding is light and not accompanied by the usual menstrual symptoms. However, the fact that you had sexual activity on the 21st and the timing of your bleeding suggests that it could also be an early onset of your menstrual cycle.
Given that you had a negative pregnancy test on the 3rd, it is also important to consider that the test may not have detected pregnancy if it occurred after the test was taken. If you suspect you might be pregnant, it would be advisable to wait a few days and take another pregnancy test, as hormone levels can increase rapidly in early pregnancy.
In conclusion, if the bleeding you are experiencing is light, pink or brown, and lasts only a short time, it could be implantation bleeding. However, if it is heavier, lasts longer, and is accompanied by typical menstrual symptoms, it is likely your period. If you continue to have concerns or if the bleeding changes in character, it would be wise to consult with a healthcare provider for further evaluation.
