Frequent urination issues?
Hello Doctor, I suddenly started experiencing frequent urination three days ago.
The day before, I was still urinating normally, about every 2 to 3 hours.
On the day the symptoms began, I took 36 mg of Concerta, and from that afternoon onwards, I needed to urinate approximately every half hour.
The urine is clear and colorless, with a volume of about 200 cc each time.
Over the past few days, I've been urinating about every 1 to 2 hours in the morning, every 30 minutes to 1 hour in the afternoon, and about every 15 minutes after dinner until bedtime.
During the night, I urinate normally, and upon waking, the first urine volume is about 300 cc, mostly clear and colorless.
I drink about 2000 to 2500 cc of water daily.
Today marks the fourth day with these symptoms, and I would like to ask the doctor what possible causes this could have and which specialty I should consult.
Thank you.

Frequent urination, also known as polyuria, can be a distressing symptom that significantly impacts daily life. In your case, the sudden onset of frequent urination after starting Concerta (a medication commonly used to treat ADHD) raises several considerations.
Firstly, it is important to understand that medications can have various side effects, and Concerta, which contains methylphenidate, is known to potentially affect the urinary system. While not a common side effect, some individuals may experience changes in urinary frequency or urgency. This could be due to the stimulant properties of the medication, which may increase metabolic rate and fluid intake, leading to increased urine production.
In your situation, the fact that you are experiencing a significant increase in urination frequency—from every 2-3 hours to every 30 minutes to an hour—suggests that there may be other underlying factors at play. The clear, colorless urine and the relatively large volume (around 200cc per void) indicate that you are likely well-hydrated, which is a positive sign. However, it is essential to consider other potential causes for your symptoms, including:
1. Diabetes Mellitus: One of the classic symptoms of diabetes is polyuria, often accompanied by increased thirst (polydipsia). If your body is unable to effectively utilize glucose, it may excrete excess glucose through urine, drawing water with it and resulting in increased urination.
2. Diuretic Effect: If you have increased your fluid intake significantly or are consuming diuretic substances (like caffeine or alcohol), this could lead to more frequent urination.
3. Urinary Tract Infection (UTI): Although your urine is clear and without signs of infection, UTIs can sometimes present with increased frequency without the typical symptoms of pain or urgency.
4. Bladder Overactivity: Conditions like overactive bladder can cause frequent urination and may be exacerbated by stress or anxiety.
5. Neurological Conditions: Since you mentioned the use of Concerta, it’s worth noting that certain neurological conditions can affect bladder control and function.
Given that your symptoms have persisted for four days, it is advisable to seek medical attention. The best course of action would be to consult a urologist, who specializes in urinary tract issues. They can perform a thorough evaluation, including a urinalysis, urine culture, and possibly imaging studies to rule out any anatomical abnormalities or infections.
In addition to urology, if you suspect that the Concerta may be contributing to your symptoms, discussing this with the prescribing physician or a psychiatrist may also be beneficial. They can assess whether the medication is appropriate for you or if adjustments are needed.
In summary, while frequent urination can be a benign issue, especially if related to increased fluid intake or medication side effects, it can also indicate more serious underlying conditions. Prompt evaluation by a healthcare professional is crucial to determine the cause and appropriate treatment. Keeping a diary of your fluid intake, urinary frequency, and any associated symptoms can also be helpful for your healthcare provider in diagnosing the issue.
