Phimosis circumcision?
I live in a rural area where there are no urologists available for direct consultation, so I would like to ask for some advice.
I previously asked a doctor but did not receive an answer, so I apologize for bothering you again! I have phimosis, which is at a level where I can see the urethral opening when I pull the foreskin down.
I understand that phimosis can lead to odor issues, and since I have a cleanliness obsession, I have developed a cleaning method (which I have been using for a long time).
However, I am unsure if it is correct, so I would like to consult with you.
My cleaning method involves using a small hose-like device to rinse towards the glans, and during the rinsing, water enters the space created between the foreskin and the glans (similar to separating the foreskin from the glans).
While rinsing, I also rub my penis up and down.
I am wondering if this method effectively achieves cleanliness.
I am considering getting a circumcision in the future, but I have heard that it can be quite painful, and I am afraid of pain.
Additionally, at my age, am I at a higher risk for penile cancer? I am a bit worried.
Lastly, I would like to ask if experiencing pain on the glans after excessive masturbation is due to overdoing it.
I had taken a break, but after a while, the pain returned.
Thank you, doctor!

Hello: Your condition is phimosis.
If the glans is fully exposed when not erect, it is recommended to consider circumcision.
Your cleaning method is acceptable, but remember to keep the area dry.
The circumcision procedure may be painful, but it is manageable as long as you are not overly sensitive to pain.
The risk of penile cancer is low.
When performing DIY methods, be careful not to apply too much force, as it will definitely cause discomfort.
Wishing you good health.

Phimosis, or the inability to retract the foreskin fully, can lead to various hygiene issues and potential health concerns. Your inquiry about cleaning methods and circumcision is quite valid, especially given your concerns about cleanliness and discomfort.
First, let's address your cleaning technique. Using a gentle stream of water to clean the area is generally acceptable, but it’s crucial to be cautious. Forcing water into the space between the foreskin and the glans (the head of the penis) can sometimes lead to discomfort or even injury if done too forcefully. It’s important to avoid any aggressive manipulation, as this can cause irritation or inflammation. Instead, a mild soap and warm water can be used to clean the external area gently. If you can retract the foreskin partially without pain, you can clean the area beneath it as well, but do so gently to avoid any tearing or discomfort.
Regarding your concern about circumcision, it’s understandable to feel apprehensive about the procedure. Circumcision is a common surgical procedure that involves the removal of the foreskin. While it can be associated with pain and discomfort post-surgery, many find that the benefits, such as improved hygiene and reduced risk of certain infections, outweigh the temporary discomfort. Pain management strategies, including medications prescribed by your doctor, can help alleviate discomfort during the recovery period.
As for your concern about penile cancer, the risk is generally low, especially in younger individuals. However, phimosis can increase the risk of infections and inflammation, which may contribute to complications if left untreated. Regular check-ups with a healthcare provider, even if it requires traveling, can help monitor your condition and address any concerns early on.
Regarding your question about discomfort after frequent masturbation, it’s not uncommon to experience sensitivity or soreness if the area is stimulated excessively. If you notice persistent pain, it may be a sign to take a break and allow your body to recover. Ensuring that you are using adequate lubrication can also help reduce friction and discomfort during masturbation.
In summary, while your cleaning method is on the right track, it’s essential to be gentle and avoid forcing water into the foreskin. If you continue to have concerns about phimosis, circumcision, or any symptoms you’re experiencing, I highly recommend consulting a urologist or healthcare provider who can provide personalized advice and treatment options. Regular monitoring and open communication with a healthcare professional can help alleviate your fears and ensure your health is prioritized.
