Erectile dysfunction
Hello doctor, I am currently 26 years old.
In recent years, I have consumed alcohol in small amounts, but I have been smoking for seven years.
I am currently serving in the military and have a habit of exercising.
Recently, I started taking medication for chronic sinusitis, which I have been on for about two to three weeks.
During this time, I have been intimate with my girlfriend.
I have the desire to have sex, but my interest seems to be low.
During foreplay, my penis does become erect, but I am unsure if this is a psychological issue or something else, as it becomes flaccid before penetration.
I have developed some anxiety about this, and no matter how much stimulation I receive afterward, I cannot achieve an erection.
Just yesterday, when I was about to be intimate, I did have an erection during foreplay and oral sex, but when it came time for penetration, it started to go soft again, roughly an hour after taking the medication.
I am unsure if the medication is the cause, as I only started experiencing these symptoms after beginning the treatment.
Below are the medications prescribed by my ENT doctor: Duran 625 mg, Pseudoephedrine 60 mg, Amsol, and a steroid nasal spray.
Based on my description, do you have any insights into the possible cause?

More Info
Erectile dysfunction (ED) is a common issue that can affect men of all ages, including those in their twenties. Based on your description, it seems that you are experiencing difficulties achieving or maintaining an erection, particularly during sexual activity with your girlfriend. This can be a distressing experience, especially when it occurs in the context of a new medication regimen for chronic sinusitis.
Possible Causes of Your Erectile Dysfunction
1. Psychological Factors: Anxiety, stress, and psychological pressure can significantly impact sexual performance. The fact that you mentioned feeling a "shadow" or anxiety about your performance after experiencing difficulties may contribute to a cycle of anxiety that exacerbates the problem. Performance anxiety is particularly common among younger men and can lead to a temporary inability to maintain an erection.
2. Medication Side Effects: The medications you are taking for chronic sinusitis, including Duran (which contains a combination of medications) and pseudoephedrine, can potentially affect sexual function. Pseudoephedrine, a decongestant, can lead to increased blood pressure and may have side effects that impact erectile function. Additionally, corticosteroid nasal sprays can also have systemic effects, although they are less likely to directly cause ED.
3. Lifestyle Factors: While you mentioned that you consume alcohol in moderation and have a smoking habit, both smoking and excessive alcohol consumption can negatively affect erectile function. Smoking can impair blood flow, which is crucial for achieving and maintaining an erection. Even moderate alcohol consumption can lead to temporary erectile difficulties, especially if combined with anxiety about performance.
4. Physical Health: Although you are young and physically active, underlying health issues can sometimes contribute to ED. Conditions such as diabetes, hormonal imbalances, or cardiovascular issues can affect erectile function. It's important to consider your overall health and any other symptoms you may be experiencing.
Recommendations for Management
1. Consult Your Healthcare Provider: Since you are experiencing these symptoms after starting a new medication, it is essential to discuss this with your healthcare provider. They can evaluate whether your current medications may be contributing to your erectile dysfunction and consider alternatives if necessary.
2. Address Psychological Factors: If anxiety or stress is a contributing factor, consider speaking with a mental health professional or a sex therapist. They can help you develop coping strategies and address any underlying psychological issues.
3. Lifestyle Modifications: Reducing or quitting smoking and moderating alcohol intake can improve overall health and potentially enhance erectile function. Regular exercise, a balanced diet, and maintaining a healthy weight can also contribute positively to sexual health.
4. Monitoring and Follow-Up: Keep track of your symptoms and any changes you notice. If the problem persists, further evaluation may be necessary. This could include blood tests to check hormone levels, blood sugar, and other relevant health markers.
5. Open Communication: Discuss your concerns with your partner. Open communication can alleviate some of the pressure you may feel during intimate moments and help both of you navigate this situation together.
Conclusion
Erectile dysfunction can stem from a variety of factors, including psychological stress, medication side effects, and lifestyle choices. Given your recent changes in medication and the psychological impact of your experiences, it is crucial to approach this issue holistically. Consulting with your healthcare provider will help you determine the best course of action and address any underlying concerns. Remember, you are not alone in this, and many men experience similar challenges at various points in their lives.
