Postoperative care for anal abscess?

I had an anal abscess surgery at the end of April 2021, and now there is still a small hole in the wound that sometimes bleeds a little and occasionally has a bit of pus.
If there is no pus, it’s just a small hole.
Is this normal? Do I need to see a doctor? Thank you, doctor.

Dr. Zhang Shuhao reply Surgery

If there are still wounds and purulent discharge, there may be concerns about an inadequately drained abscess or the formation of an anal fistula.
It is advisable to return to the original surgeon for a follow-up, explain the current situation, and assess whether medication or surgical treatment is necessary.

After undergoing surgery for an anal abscess, it is not uncommon to have some residual drainage or a small opening at the surgical site. However, the presence of ongoing bleeding, discharge, or a persistent opening can indicate that the healing process is not proceeding as expected. Here are some important considerations regarding post-operative care for an anal abscess and when to seek medical attention.

Normal Healing Process
1. Initial Recovery: After surgery, it is typical for the surgical site to have some drainage as the body works to heal. This drainage can be a mix of blood, pus, and other fluids. In the first few days to weeks, some discharge is expected, especially if the abscess was large or if there was significant tissue involvement.

2. Signs of Healing: Over time, the drainage should decrease, and the wound should begin to close. The presence of a small opening (fistula) may remain for a while, but it should gradually diminish in size and frequency of discharge.

When to Seek Medical Attention
You should consider seeking medical attention if you experience any of the following:
1. Persistent or Increasing Discharge: If the discharge from the wound continues to be significant or increases in volume, this may indicate an infection or that the abscess has not fully resolved.

2. Foul Odor: A foul smell from the wound can be a sign of infection. If you notice this, it is important to consult your healthcare provider.

3. Bleeding: While some minor bleeding can occur, especially if the area is irritated, persistent or heavy bleeding is a concern and should be evaluated by a doctor.

4. Pain: If you experience increasing pain at the surgical site, this could indicate complications such as infection or abscess recurrence.

5. Fever or Chills: Systemic symptoms like fever or chills can indicate a more serious infection and warrant immediate medical attention.

6. Swelling or Redness: Increased swelling or redness around the surgical site can also be a sign of infection or complications.

Recommended Actions
- Follow-Up Appointments: It is crucial to attend all scheduled follow-up appointments with your surgeon or healthcare provider. They can assess the healing process and determine if any further intervention is necessary.

- Wound Care: Keep the area clean and dry. Follow your surgeon's instructions regarding wound care, including any recommended cleaning solutions or dressings.

- Monitor Symptoms: Keep track of any changes in symptoms, including the amount and type of discharge, pain levels, and any new symptoms that arise.

- Pain Management: If you are experiencing discomfort, over-the-counter pain relievers may help, but consult your doctor for recommendations specific to your situation.

Conclusion
While some drainage and a small opening can be normal after anal abscess surgery, it is essential to monitor your symptoms closely. If you notice any concerning signs such as persistent bleeding, increasing pain, or foul discharge, do not hesitate to contact your healthcare provider. Early intervention can prevent complications and promote better healing outcomes. Always prioritize your health and well-being by staying informed and proactive about your recovery.
