Should I get a circumcision for phimosis?

Hello doctor, I have a long foreskin.
Initially, I could barely manage to keep the glans covered during an erection, but then I developed balanitis, which caused the foreskin to become loose.
Now that the inflammation has resolved, I am experiencing lymphangitis of the penis, which is not painful or itchy, but there is swelling on the left side of the penis.
Although I can retract the foreskin for cleaning, the glans does not become exposed during an erection, and it cannot be held in place.
During intercourse, it often feels like the foreskin is doing all the work, leading to a lack of sensation and difficulty maintaining an erection.
I usually have to use my hand to achieve an erection and ejaculation more easily.
Should I consider a circumcision in this situation? Thank you.

Hello: If your situation involves circumcision to improve sexual performance, it may not necessarily help; it can only be said that there is a possibility of improvement.
Wishing you good health.

When considering circumcision for long foreskin issues, it is essential to evaluate the specific symptoms and conditions you are experiencing. Based on your description, it seems that you are dealing with a long foreskin that has led to complications such as balanitis (inflammation of the glans) and lymphangitis (inflammation of the lymphatic vessels in the penis). These conditions can be uncomfortable and may affect your sexual function.

Circumcision is a surgical procedure that involves the removal of the foreskin, which can provide several benefits, especially in cases where the foreskin is excessively long or causes recurrent infections. Here are some factors to consider regarding your situation:
1. Recurrent Infections: If you have experienced recurrent balanitis, it may indicate that the long foreskin is contributing to hygiene issues. A longer foreskin can trap moisture and bacteria, leading to inflammation and infection. Circumcision can help alleviate this problem by removing the foreskin and making it easier to maintain hygiene.

2. Sexual Function: You mentioned that during erection, the foreskin does not retract adequately, which can lead to discomfort during sexual activity. This can significantly impact your sexual experience and satisfaction. Circumcision may improve sexual function by allowing for better exposure of the glans and reducing friction that can occur with a long foreskin.

3. Lymphangitis: While you noted that the lymphangitis is not painful or itchy, it is still a sign of inflammation that may be related to the long foreskin. Addressing the underlying issue by considering circumcision could potentially reduce the risk of further complications.

4. Personal Preference and Cultural Considerations: Circumcision is a personal decision that can be influenced by cultural, religious, or personal beliefs. It is important to weigh these factors alongside the medical considerations.

5. Consultation with a Specialist: Before making a decision, it is crucial to consult with a urologist or a healthcare provider who specializes in male reproductive health. They can provide a thorough examination, discuss the potential risks and benefits of circumcision, and help you understand what to expect from the procedure.

6. Postoperative Care and Recovery: If you decide to proceed with circumcision, be aware that there will be a recovery period during which you will need to follow specific care instructions to ensure proper healing and minimize the risk of complications.

In conclusion, based on your symptoms and the impact they have on your quality of life, circumcision may be a viable option to consider. It could potentially resolve the issues related to hygiene, recurrent infections, and sexual function. However, it is essential to have an in-depth discussion with a healthcare professional to explore all your options and make an informed decision that aligns with your health needs and personal circumstances.
