Tibialis anterior muscle swelling
Hello, doctor.
About two days ago, I noticed swelling in my calf (anterior and lateral sides) that is painful when pressed.
The next day, I found a small protruding spot in the swollen area, and now it only hurts when I press that spot.
The whole process feels similar to developing a pimple.
I would like to ask if there is a possibility of a tumor, and whether I should see a doctor soon, or if I can wait and see if it resolves on its own.
Additionally, two of my family members have gout, and I noticed that during the two days of swelling, I had eaten a lot of seafood.
I would like to know if there is a possibility of gout as well.
Thank you.

Dr. Chen Yongxue reply Orthopedics
Hello, it appears to be inflammation of the tibialis anterior muscle.
I recommend monitoring the situation for now.
If there is no improvement after two weeks, you may follow up at an orthopedic clinic.
Thank you, it is not gout.

More Info
Understanding anterior shin swelling can be quite concerning, especially when accompanied by pain and changes in the skin. Based on your description, it seems you are experiencing swelling on the anterior and lateral aspects of your lower leg, with a specific painful point that resembles a pimple. This situation raises several possibilities regarding the underlying causes of your symptoms.
Possible Causes of Anterior Shin Swelling
1. Trauma or Injury: If you've recently engaged in physical activities or experienced any trauma to the area, this could lead to localized swelling and pain. Even minor injuries can result in inflammation.
2. Infection: The presence of a painful, raised area could indicate a localized infection, such as a skin abscess or folliculitis. If the area is warm, red, or increasingly painful, this would warrant immediate medical attention.
3. Gout: Given your family history of gout and recent dietary choices (increased seafood consumption), it is plausible that you could be experiencing a gout flare-up. Gout typically presents with sudden, severe pain and swelling in the joints, often starting in the big toe, but it can affect other areas, including the shin.
4. Tendonitis or Bursitis: Inflammation of the tendons or bursae around the shin can also cause swelling and pain. This is often related to overuse or repetitive strain.
5. Tumors or Cysts: While less common, the possibility of a benign tumor (such as a lipoma) or a cyst (like a ganglion cyst) should not be ruled out, especially if the swelling persists or grows.
When to Seek Medical Help
Given the symptoms you've described, it is advisable to consult a healthcare professional, particularly if:
- The swelling does not improve or worsens over the next few days.
- You experience increased pain, redness, or warmth in the area.
- You develop fever or systemic symptoms (like chills or malaise).
- You notice any changes in mobility or function of the leg.
- The painful area becomes larger or more prominent.
Observation vs. Immediate Action
While it may be tempting to wait and see if the swelling resolves on its own, the presence of pain and the specific characteristics of the swelling suggest that a medical evaluation would be prudent. Early diagnosis and treatment can prevent potential complications, especially if an infection or gout is involved.
Conclusion
In summary, while there are several potential causes for your anterior shin swelling, including trauma, infection, gout, and other conditions, it is essential to monitor your symptoms closely. Given your family history of gout and recent dietary changes, this could be a contributing factor. However, the presence of a painful, raised area also raises concerns for infection or other issues that may require medical intervention. Therefore, I recommend scheduling an appointment with your healthcare provider to discuss your symptoms in detail and receive appropriate evaluation and treatment.
