Blurry vision upon waking up?

Hello Doctor: I have dry eye syndrome and high myopia.
When I turn on the lights in the morning, I notice that my vision is blurry (it feels foggy) for about 3 to 5 minutes until it gradually returns to normal after I wash my face.
Could you please explain the reasons for experiencing blurry vision in the morning? Do I need to see an ophthalmologist immediately? Thank you very much for your guidance.

Youna, 40~49 year old female. Ask Date: 2022/12/05

Dr. Gao Pilin reply Ophthalmology

Hello: It could be due to dry eyes or the presence of discharge, which may cause blurriness.
After washing your face, your vision gradually returns to normal.
However, it could also be related to floaters; upon waking, the floaters may settle down, leading to clearer vision.
It is likely not a concern, but it is still advisable to have it checked.

More Info

Experiencing blurry vision upon waking up can be a common issue, especially for individuals with pre-existing conditions such as dry eye syndrome and high myopia. Let's explore the potential reasons behind this phenomenon and whether you should seek immediate medical attention.

Causes of Blurry Vision Upon Waking
1. Dry Eye Syndrome: Given that you have dry eye syndrome, this could be a significant factor in your morning blurry vision. During sleep, tear production decreases, leading to dryness. When you wake up, your eyes may not have sufficient moisture, causing a temporary blurriness until you blink enough to rehydrate the surface of your eyes.

2. Corneal Edema: Overnight, the cornea can absorb excess fluid, especially if you sleep with your eyes partially open or if your eyelids do not close completely. This can lead to corneal swelling (edema), resulting in blurry vision that typically resolves after a few minutes as the cornea returns to its normal state.

3. Visual Acuity Adjustment: Upon waking, your eyes may need time to adjust to the light and focus properly. This adjustment period can lead to temporary blurriness as your eyes transition from a dark environment to a well-lit one.

4. High Myopia: Individuals with high myopia may experience more pronounced visual disturbances upon waking. The structural changes in the eye associated with high myopia can make the eyes more sensitive to changes in light and focus, contributing to temporary blurriness.

5. Sleep Position and Eye Pressure: The position in which you sleep can also affect your eyes. If you sleep on your side or face down, this may put pressure on your eyes, leading to temporary visual disturbances upon waking.

When to Seek Medical Attention
While occasional blurry vision upon waking is often benign, there are circumstances where you should consider consulting an eye care professional:
- Persistent Symptoms: If the blurry vision lasts longer than a few minutes or occurs frequently, it may warrant further investigation.

- Accompanying Symptoms: If you experience other symptoms such as pain, redness, significant vision loss, or flashes of light, you should seek immediate medical attention.

- Changes in Vision: Any sudden changes in your vision that are different from your usual experience should be evaluated by an eye care professional.

Recommendations
1. Hydration and Eye Drops: Consider using lubricating eye drops before bed to help maintain moisture in your eyes overnight. This can alleviate some of the dryness that contributes to blurry vision in the morning.

2. Sleep Hygiene: Ensure that you are getting adequate sleep and consider your sleep position. If you suspect that your position may be affecting your eyes, try sleeping on your back.

3. Regular Eye Exams: Continue with your regular eye examinations, especially given your history of dry eye and high myopia. Your eye care provider can monitor your condition and make recommendations tailored to your needs.

4. Consultation: If your symptoms persist or worsen, do not hesitate to reach out to your eye care provider for a thorough evaluation. They can assess your eye health and determine if any underlying issues need to be addressed.

In conclusion, while blurry vision upon waking can be attributed to several benign factors, it is essential to monitor your symptoms and seek professional advice if necessary. Taking proactive steps to manage your dry eye syndrome and maintaining regular check-ups can help ensure your eye health remains stable.
