One month after root canal treatment, I am still unable to bite down on anything?
Hello Doctor: I underwent root canal treatment on my lower left molar at the beginning of November.
It has been almost a month since the procedure, but I still experience pain when applying pressure or tapping on the tooth, and I cannot bite down on anything without pain.
Additionally, although I have no sensation to hot or cold after the nerve was removed, I sometimes feel a dull sensation in the tooth when drinking hot water or soup.
It’s not painful, but it feels somewhat numb.
I mentioned this to my doctor during my follow-up appointment, and after taking an X-ray, the doctor confirmed there were no fractures and suggested monitoring the situation.
If it persists for another two months, I may need to undergo microscopic root canal treatment.
1.
Why do I still feel a dull sensation in the tooth when drinking hot water after the nerve has been removed? (I am very certain that the sensation is coming from that tooth.)
2.
How long should I monitor the situation regarding the pain when biting down? Thank you!

Hello: Treatment not being successful is a possible outcome, and observation for about six months may be warranted.
If discomfort persists beyond six months, you might consider undergoing another root canal treatment or checking for other issues such as periodontal inflammation, malocclusion, or trigeminal neuralgia.

Persistent pain after a root canal treatment can be concerning and frustrating. It’s important to understand the potential reasons behind your symptoms and the appropriate next steps to take.
1. Why do I feel a dull sensation in my tooth when drinking hot liquids, even after the nerve has been removed?
The sensation you are experiencing, even after the nerve has been removed, can be attributed to several factors. First, the tooth may still have some residual inflammation in the surrounding tissues, which can lead to discomfort when exposed to temperature changes. The pulp tissue, although removed, can leave behind some nerve endings that may still react to thermal stimuli. Additionally, the materials used in the root canal procedure, such as the filling material, can sometimes cause a sensation of pressure or dullness when exposed to heat. This is not uncommon, and many patients report similar experiences after root canal treatments.
2. How long should I observe the pain when biting down?
The pain you are experiencing when biting down on that tooth should ideally improve within a few weeks after the procedure. However, if the pain persists beyond a month, as in your case, it is advisable to follow up with your dentist. Your dentist may recommend a period of observation, typically around 2 months, as you mentioned. If the pain continues after this period, further investigation may be warranted, such as a microscopic examination of the root canal system, which could reveal any missed canals or complications that might not have been evident on the initial X-rays.
In the meantime, here are some additional steps you can take to manage your symptoms:
- Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate discomfort. However, consult your dentist before taking any medication, especially if you have other health conditions or are on other medications.
- Dietary Adjustments: Avoid hard or chewy foods that may exacerbate the pain when biting. Stick to softer foods until the discomfort subsides.
- Follow-Up Appointments: Keep your scheduled follow-up appointments with your dentist. If your symptoms worsen or new symptoms arise, such as swelling or fever, contact your dentist immediately.
- Consider a Specialist: If your symptoms do not improve, your dentist may refer you to an endodontist, a specialist in root canal treatments, for further evaluation and potential retreatment.
In conclusion, while some discomfort can be expected after a root canal, persistent pain warrants further investigation. It’s essential to communicate openly with your dentist about your symptoms and follow their recommendations for monitoring and potential treatment options. Your oral health is crucial, and addressing these issues promptly can help prevent further complications.
