Postpartum hemorrhage
Hello, my last menstrual period was from October 27 to November 2, and this month it was from December 6 to December 11.
On the 11th, I had unprotected intercourse without ejaculation, and about an hour later, I took emergency contraception.
On the 17th, I experienced bleeding (approximately the amount typical for day 3 of menstruation) that lasted for about 3 to 4 days.
Is this my menstrual period, or could it be ovulation bleeding, or bleeding caused by the emergency contraception? Given the bleeding, do I still need to take a pregnancy test for confirmation?
Niming, 20~29 year old female. Ask Date: 2022/12/22

Hello, the bleeding you are experiencing cannot be determined whether it is caused by the emergency contraceptive pill or other reasons.
Additionally, the effectiveness of emergency contraception is not 100%, with a failure rate of about 3%.
It is recommended that if your period is more than a week late, you should check for pregnancy.
Thank you! Wishing you peace and health.

Post-coital bleeding can be a concerning symptom for many individuals, and understanding its potential causes is essential for proper management. Based on your timeline and the details provided, let's break down the situation.
You mentioned that your last menstrual period was from October 27 to November 2, and your next period was from December 6 to December 11. After having unprotected intercourse on November 11 and taking emergency contraception (the "morning-after pill") shortly afterward, you experienced bleeding around November 17, which lasted for about 3 to 4 days and was similar in volume to your menstrual flow.
There are several possibilities for the bleeding you experienced:
1. Withdrawal Bleeding from Emergency Contraception: Emergency contraceptive pills (ECPs) can cause changes in your menstrual cycle. They often lead to withdrawal bleeding, which can occur a few days to a few weeks after taking the pill. This bleeding can resemble a menstrual period but may be lighter or shorter in duration. Given that you took the ECP shortly after unprotected intercourse, it is plausible that the bleeding you experienced on November 17 was a result of the hormonal changes induced by the medication.
2. Ovulation Bleeding: If your cycle is relatively short, ovulation can occur around the middle of your cycle, which may lead to spotting or light bleeding. However, this typically occurs about 14 days before your next expected period, which in your case would be around November 22. Since your bleeding occurred earlier, it is less likely to be ovulation bleeding.
3. Menstrual Period: The bleeding you experienced could also be an early onset of your next menstrual period. However, since it occurred shortly after taking the emergency contraceptive, it is more likely to be related to that.
4. Other Causes: While less common, post-coital bleeding can also be caused by cervical or vaginal irritation, infections, or other underlying conditions. If the bleeding was accompanied by pain, unusual discharge, or if it continues, it would be advisable to consult a healthcare provider.
Regarding your question about whether you should take a pregnancy test: If you had unprotected intercourse and are concerned about the possibility of pregnancy, it is always a good idea to take a pregnancy test, especially if your next expected period is late or if you experience any unusual symptoms. Most home pregnancy tests are reliable if taken at least a week after the missed period.
In summary, the bleeding you experienced after taking emergency contraception is likely a withdrawal bleed due to the hormonal changes from the medication. However, if you have any concerns or if the bleeding continues or changes in nature, it is essential to consult with a healthcare provider for further evaluation and guidance. They can provide personalized advice based on your medical history and current symptoms.
