The sensation during urination feels as sensitive as foreplay?
Over the past few months, I have experienced a sensation of urgency whenever I feel the need to urinate, accompanied by an overwhelming feeling that I cannot hold it in.
If I rush to the bathroom, I often end up urinating in my pants uncontrollably.
Therefore, when I feel the urge, I usually have to hold it in until the sensation passes before I can safely go to the restroom.
In addition to this issue, for the past month or so, I have developed another concern: every time I urinate, I experience a warm, sensitive, and stimulating sensation, similar to foreplay.
I would like to know what might be causing these symptoms.
To provide some background, I have had a tendency to experience lower body edema since childhood, which led me to drink very little water.
I have generally felt fine until recently when people around me suggested that I should hydrate properly.
Consequently, I started drinking about 1200 ml of water daily, which resulted in my current issue of uncontrollable urination, along with the sensitivity.
Over a week ago, I returned to my normal water intake (which has always been just a few sips when thirsty, and not drinking when not thirsty), but the problems persist without any change.
I feel that my initial decision to increase my water intake has caused these issues for me.

Hello: Have you ever had a urinalysis? Have you experienced any urinary tract infections? Have you given birth vaginally? I suggest that you have a "major issue" since you are still young.
You should go to the hospital for a stool test, arrange a voiding diary for three days, and receive appropriate oral medication or education (Kegel exercises).
I hope you recover soon.

More Info
Understanding urinary sensitivity and discomfort can be quite complex, especially when it involves symptoms like urgency, incontinence, and unusual sensations during urination. Based on your description, it seems you are experiencing a combination of urinary urgency, incontinence, and heightened sensitivity in the genital area, which can be distressing and disruptive to daily life.
Causes of Your Symptoms
1. Overactive Bladder (OAB): The feeling of urgency and the inability to hold urine can be indicative of an overactive bladder. This condition is characterized by a sudden urge to urinate that may be difficult to control. It can lead to involuntary loss of urine, which seems to align with your experience of wetting yourself when you feel the urge to go.
2. Urinary Tract Infection (UTI): Although you did not mention any specific symptoms of a UTI, such as burning during urination or cloudy urine, it’s important to consider that infections can cause increased sensitivity and urgency. UTIs can irritate the bladder and urethra, leading to discomfort and frequent urges to urinate.
3. Pelvic Floor Dysfunction: The pelvic floor muscles play a crucial role in bladder control. If these muscles are weak or not functioning properly, it can lead to issues with urinary retention and urgency. Stress, anxiety, or even physical strain can exacerbate these symptoms.
4. Fluid Intake and Retention: You mentioned that you have a history of lower body edema and have recently increased your water intake. While staying hydrated is essential, excessive fluid intake can sometimes lead to increased urinary frequency, especially if your body is not used to processing that amount of fluid. Conversely, inadequate hydration can lead to concentrated urine, which may irritate the bladder.
5. Nerve Sensitivity: The heightened sensitivity you described, particularly the "hot" sensation during urination, could be related to nerve sensitivity or irritation in the pelvic region. This can occur due to various factors, including inflammation or previous infections.
Recommendations for Management
1. Consult a Urologist: Given the complexity of your symptoms, it is crucial to consult a urologist. They can perform a thorough evaluation, including urine tests, imaging studies, and possibly urodynamic testing to assess bladder function and rule out any underlying conditions.
2. Bladder Training: This involves gradually increasing the time between urination to help train your bladder to hold more urine. Start by trying to hold off for a few minutes longer than your usual urge, gradually increasing the interval.
3. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels can improve bladder control. A physical therapist specializing in pelvic health can provide guidance on proper techniques.
4. Dietary Adjustments: Monitor your fluid intake and consider reducing irritants such as caffeine, alcohol, and spicy foods, which can exacerbate bladder sensitivity. Ensure you are drinking enough water to stay hydrated without overdoing it.
5. Medication: If your symptoms are severe, medications that relax the bladder or help manage overactive bladder symptoms may be prescribed by your healthcare provider.
6. Stress Management: Since anxiety can exacerbate urinary symptoms, consider incorporating stress-reduction techniques such as mindfulness, yoga, or counseling.
Conclusion
Your symptoms of urinary urgency, incontinence, and heightened sensitivity are multifaceted and may stem from a combination of factors, including bladder dysfunction, nerve sensitivity, and possibly previous infections. It is essential to seek professional medical advice for an accurate diagnosis and tailored treatment plan. With the right approach, many individuals find significant relief from their symptoms and can regain control over their urinary health.
