When can pregnancy be confirmed?

Hello Doctor: My menstrual cycle has always been irregular.
My last period started on October 24, and I visited the doctor who prescribed medication (I forgot the name of the medication) to be taken twice a day.
The doctor mentioned that my menstrual cycle would likely restart around the end of the month.
Since my husband and I have decided to try for a baby, we had unprotected intercourse on October 27.
As of today, my period has not yet arrived.
I am experiencing some lower abdominal pain and bloating, similar to what I feel before my period, and my breasts are also tender.
When can I confirm if I am pregnant? Can I have a Pap smear done now? Additionally, about two years ago, I was diagnosed with hypothyroidism and had surgery to remove a thyroid cyst.
Is there anything special I should be aware of during pregnancy? Thank you for your response.

DALE, 20~29 year old female. Ask Date: 2003/11/02

Hello! 1.
You can take a pregnancy test 2-3 weeks after sexual intercourse.
2.
A Pap smear should be performed when there is no bleeding and no inflammation (such as abnormal discharge); it is generally recommended to do it a few days after the end of menstruation.
3.
You can return to the endocrinology department for blood tests to monitor thyroid function; if the results are normal, you can conceive.
Sincerely, Dr.

To confirm a pregnancy, the timing and method of testing are crucial. Generally, a pregnancy can be confirmed through a home pregnancy test or a blood test at a healthcare provider's office. Home pregnancy tests detect the hormone human chorionic gonadotropin (hCG), which is produced shortly after a fertilized egg attaches to the uterine lining. This hormone can typically be detected in urine about one week after a missed period, which is usually around two weeks after conception. However, since your menstrual cycle is irregular, it may be beneficial to wait at least a week after your expected period to take a home pregnancy test for more reliable results.

In your case, since you had unprotected intercourse on October 27 and your period has not yet arrived, you may consider taking a home pregnancy test around November 3 or 4. If the test is positive, it would be advisable to schedule an appointment with your healthcare provider for a blood test to confirm the pregnancy and to discuss your health history, especially considering your previous thyroid issues.

Regarding the symptoms you are experiencing, such as lower abdominal pain and breast tenderness, these can be early signs of pregnancy, but they can also be related to other factors, including hormonal changes or premenstrual symptoms. It’s important to note that while some women experience these symptoms early in pregnancy, they are not definitive indicators of pregnancy on their own.

As for the Pap smear (cervical smear) test, it is generally recommended to have this test done during routine gynecological exams, but it is not specifically used to confirm pregnancy. If you are due for a Pap smear based on your healthcare provider's recommendations, you can discuss this during your appointment. However, if you are currently experiencing symptoms that could indicate pregnancy, it might be best to confirm the pregnancy first before proceeding with the Pap test.

Given your history of hypothyroidism and previous thyroid surgery, it is essential to monitor your thyroid levels closely during pregnancy. Thyroid hormones play a critical role in fetal development, particularly in the first trimester. If you become pregnant, your healthcare provider may adjust your thyroid medication dosage to ensure that both you and your baby remain healthy. Regular check-ups will be necessary to monitor your thyroid function and overall health throughout the pregnancy.

In summary, to confirm your pregnancy, wait until at least a week after your expected period to take a home pregnancy test. If positive, follow up with your healthcare provider for further testing and guidance. Discuss your symptoms and any concerns regarding your thyroid health during your appointment. It’s essential to maintain open communication with your healthcare provider to ensure a healthy pregnancy.
