Hit by a ball on the left side of the ribcage?
Hello, I was hit in the left side of my ribcage by a softball that bounced off the ground while playing softball.
If my rib is not fractured, is it possible for my spleen to rupture from the impact? Or does the rib need to be fractured for the spleen to be at risk of rupture?
Niming, 20~29 year old female. Ask Date: 2022/12/26
Dr. Chen Ronghong reply Surgery
Hello, a splenic rupture often accompanies left rib fractures; however, the absence of fractures does not guarantee that the spleen is unharmed.
Therefore, it is important to be aware of the symptoms of splenic hemorrhage, which include hypotension (weakness and fatigue) or pain in the left upper abdomen.
If any of these symptoms occur, it is crucial to seek emergency medical attention immediately.

A rib injury from a softball impact can potentially lead to a splenic rupture, but the likelihood depends on several factors, including the force of the impact, the condition of the rib cage, and the individual’s anatomy.
The spleen is located in the upper left quadrant of the abdomen, just beneath the rib cage. It is somewhat protected by the ribs, but it is still vulnerable to trauma, especially if the impact is significant. When a rib is injured, it can cause pain and discomfort, but it does not necessarily mean that the spleen is damaged. However, if the impact is strong enough, it can lead to splenic injury or rupture, even if the ribs remain intact.
In general, a direct blow to the rib cage can cause a splenic rupture if the force is sufficient to transmit energy to the spleen. This can occur without rib fractures, especially in cases of blunt trauma. Factors that increase the risk of splenic injury include:
1. Force of Impact: A high-velocity impact, such as a hard throw or a collision, can cause significant trauma to the spleen.
2. Existing Conditions: Certain medical conditions, such as splenomegaly (enlargement of the spleen), can make the spleen more susceptible to injury.
3. Age and Health: Younger individuals or those with healthy spleens may have a lower risk of rupture compared to older adults or those with underlying health issues.
Symptoms of a splenic rupture can include severe abdominal pain, tenderness in the left upper quadrant, signs of internal bleeding (such as dizziness, fainting, or low blood pressure), and sometimes referred pain to the left shoulder (Kehr's sign). If you experience any of these symptoms following a rib injury, it is crucial to seek medical attention immediately.
In your case, since you mentioned being hit on the left side of the rib cage, it is essential to monitor for any signs of complications. If you experience worsening pain, difficulty breathing, or any of the symptoms mentioned above, you should consult a healthcare professional for further evaluation. Imaging studies, such as an ultrasound or CT scan, may be necessary to assess for any internal injuries, including a splenic rupture.
In summary, while a rib injury from a softball impact can potentially lead to a splenic rupture, it is not a guaranteed outcome. The severity of the impact and individual factors play a significant role in determining the risk. Always err on the side of caution and seek medical advice if you have concerns about your injury.
