Hearing louder sounds or higher frequency noises in the left ear can result in a sensation of tinnitus?

Hello Doctor, my current condition is as follows: 1.
In enclosed spaces, such as a market with loud vendor calls, my left ear feels like it hears a distortion, as if the sound is exploding inside my ear.
2.
When I hear the reversing alarm of a delivery truck in a narrow alley or suddenly hear firecrackers, my left ear also experiences this distortion.
3.
When showering for an extended period, especially while washing my hair, I can hear the resonance of the water stream hitting my scalp in my left ear.
4.
Eating in a smaller space, like a room, causes my left ear to perceive an echo when metal utensils strike ceramic bowls.
5.
When riding a bike at higher speeds or while blow-drying my hair, I notice a difference in the wind noise between my left and right ears.
These issues have been occurring for about a year, and I do not experience any problems in open spaces; for example, in a pedestrian plaza, even with various overlapping loud sounds, I do not feel any distortion.
Additionally, using earplugs or headphones effectively eliminates these issues.
I would like to ask the doctor what kind of problem this might be.

Hello, exposure to loud sounds may trigger the contraction of the stapedius muscle in the middle ear to protect the inner ear from potential damage.
Some individuals who are more sensitive may noticeably experience this phenomenon.
If the symptoms persist or if there are significant changes in hearing, it is advisable to seek medical evaluation at an otolaryngology department.

Understanding unusual sound perception in the left ear can be quite complex, especially when the symptoms have persisted for an extended period, as in your case. Based on the details you've provided, several potential causes and explanations can be considered.

1. Eustachian Tube Dysfunction: One of the most common reasons for experiencing abnormal sounds in the ear, especially in response to loud noises or changes in pressure, is Eustachian tube dysfunction. The Eustachian tube connects the middle ear to the back of the throat and helps equalize pressure. If this tube is not functioning properly, it can lead to a sensation of fullness, popping, or abnormal sound perception when exposed to loud environments or during activities that change pressure, such as washing your hair or riding a bike.

2. Middle Ear Muscle Reflex: The middle ear contains tiny muscles that contract in response to loud sounds to protect the inner ear from damage. This reflex can sometimes be overly sensitive in certain individuals, leading to a sensation of "breaking" or distortion of sound when exposed to loud noises. This could explain why you experience these sensations in confined spaces or when there are sudden loud noises.

3. Acoustic Reflex: Similar to the middle ear muscle reflex, the acoustic reflex is a protective mechanism that dampens the movement of the ossicles (tiny bones in the middle ear) in response to loud sounds. If this reflex is not functioning correctly, it may lead to unusual sound perceptions, such as the "exploding" sound you describe.

4. Tinnitus: Although you mentioned that you do not experience persistent ringing or buzzing, it is worth noting that some forms of tinnitus can manifest as unusual sound perceptions in response to external stimuli. This could be a temporary phenomenon triggered by environmental factors.

5. Auditory Processing Issues: In some cases, the brain's processing of sound can be altered, leading to unusual perceptions. This is less common but could be a factor if other medical evaluations do not reveal any physical abnormalities.

6. Physical Blockage or Earwax: While you mentioned that your ear examinations have been normal, it is still possible that a small amount of earwax or other debris could be affecting sound transmission in your left ear. This can sometimes lead to distorted sound perceptions, especially in response to specific frequencies.

7. Environmental Factors: The acoustics of the environment can also play a significant role in how sound is perceived. In smaller, enclosed spaces, sound waves can bounce off surfaces, creating echoes or amplifying certain frequencies, which may explain the sensations you experience while eating or in the shower.

Given that wearing earplugs or headphones alleviates your symptoms, it suggests that the issue may be related to how sound is processed or transmitted in your left ear rather than a structural problem.

Recommendations:
- Consult an Audiologist: Since your symptoms have persisted for a year, it would be beneficial to see an audiologist for a comprehensive hearing evaluation, including tests for Eustachian tube function and acoustic reflexes.

- Consider Environmental Modifications: If possible, try to minimize exposure to loud environments or use ear protection when you know you will be in such situations.

- Follow-Up with an ENT Specialist: If the audiologist finds no issues, a follow-up with an ENT specialist may be warranted to explore other potential causes, including any neurological factors.

In conclusion, while your symptoms can be concerning, they are not uncommon and can often be managed effectively with the right approach. Understanding the underlying mechanisms can help you navigate your experiences and seek appropriate care.
