Recovery time after removal of intramedullary nail from the tibia?

Doctor, I had a tibial fracture in my right leg at the end of March last year and had an intramedullary nail inserted.
I have recovered well and can run now.
I plan to have the intramedullary nail removed in February.
I would like to know how many days I should rest after the removal before I can walk normally again.
Additionally, I have a larger scar at the knee where the intramedullary nail was placed, which has developed into a keloid, causing discomfort when I kneel for a short time.
Is there a solution for this? Thank you.

Dr. Shi Guozheng reply Orthopedics

Approximately two weeks after the removal of the bone screws, you can start walking.
During the screw removal procedure, you can ask the physician to excise the keloid and then re-suture the area.
Afterward, it is important to implement some measures to prevent recurrence.

Recovery after the removal of a tibial intramedullary nail can vary significantly from person to person, depending on several factors including the extent of the original injury, the healing process, and individual health conditions. Generally, after the removal of the intramedullary nail, patients can expect a recovery period that may range from a few days to several weeks before they can return to normal activities, including walking without assistance.

What to Expect After Nail Removal
1. Immediate Post-Operative Care: After the removal of the intramedullary nail, you will likely experience some pain and swelling at the surgical site. This is normal and can be managed with prescribed pain medications and ice therapy. Your doctor may recommend elevating the leg to reduce swelling.

2. Weight Bearing: Most patients are encouraged to gradually increase weight-bearing activities. Initially, you may be advised to use crutches or a walker for support. Depending on your recovery and the surgeon's recommendations, you might be able to start putting weight on the leg within a few days post-surgery. However, full weight-bearing might take longer, typically around 1-2 weeks, depending on your comfort level and the surgeon's advice.

3. Physical Therapy: Engaging in physical therapy is crucial for regaining strength and mobility after the removal of the nail. A physical therapist can guide you through exercises that will help improve your range of motion and strengthen the muscles around your knee and tibia. This process can also help alleviate discomfort associated with scar tissue or keloids.

4. Keloid Management: You mentioned experiencing discomfort due to a keloid at the surgical site. Keloids can be challenging, but there are several options for management:
- Silicone Gel Sheets: These can be applied to the keloid to help flatten it over time.

- Steroid Injections: Your doctor may recommend corticosteroid injections to reduce inflammation and flatten the keloid.

- Laser Therapy: This can be effective in reducing the appearance of keloids and improving skin texture.

- Surgical Removal: In some cases, if the keloid is particularly bothersome, surgical removal may be considered, but this can sometimes lead to the formation of another keloid.

5. Return to Activities: Most patients can return to normal activities, including walking, within a few weeks after the removal of the nail. However, high-impact activities such as running or jumping may require a longer recovery period, often around 6-12 weeks, depending on individual healing and physical therapy progress.

6. Follow-Up Appointments: Regular follow-ups with your orthopedic surgeon are essential to monitor your healing process and address any concerns, including pain management and the condition of the surgical site.

Conclusion
In summary, after the removal of a tibial intramedullary nail, expect a recovery period that can range from a few days to several weeks before you can walk normally. Engage in physical therapy to aid your recovery and address any discomfort from keloids with appropriate treatments. Always consult your orthopedic surgeon for personalized advice and follow-up care to ensure a smooth recovery process.
