There is a feeling of fullness and discomfort in the lower right abdomen, along with soreness in the right lower back, and there is an unusual odor in the discharge?

Hello, doctor.
Last week during intercourse, I noticed a sharp pain in my lower right abdomen near the groin area due to impact.
However, it does not hurt when there is no impact.
The next day, I felt an unexplained bloating and heaviness in my lower right abdomen, accompanied by a sense of pressure.
My lower right back also feels sore and heavy.
When standing for a long time, my right pelvis feels painful.
Recently, my discharge has not been much, but occasionally, if there is discharge, it has a strange odor.
The feeling of heaviness and bloating is somewhat relieved when lying down, and the symptoms become more pronounced after eating.
I would like to ask if these symptoms could be indicative of pelvic inflammatory disease or other gynecological issues.
(Additional information: I have been taking Yaz for 3 years.)

Understanding right lower abdominal pain and unusual discharge in women can be complex, as various factors can contribute to these symptoms. Based on your description, it seems that you are experiencing a combination of localized pain, pressure sensations, and changes in discharge, which could indicate several potential issues.

Firstly, the right lower abdominal pain, especially when associated with sexual activity, could be related to several conditions. One possibility is that you may be experiencing pain due to pelvic inflammatory disease (PID), which is an infection of the female reproductive organs. PID can result from sexually transmitted infections (STIs) or other bacterial infections, leading to inflammation and pain in the pelvic region. Symptoms of PID often include lower abdominal pain, unusual discharge, and sometimes fever or painful urination. The presence of an unusual odor in your discharge could also suggest an infection, which is common in cases of PID.

Another potential cause of your symptoms could be ovarian issues, such as ovarian cysts or even endometriosis. Ovarian cysts can cause pain, especially if they rupture or if there is torsion (twisting) of the ovary. Endometriosis, a condition where tissue similar to the lining inside the uterus grows outside of it, can also lead to significant pain, particularly during intercourse and menstruation. The pressure and discomfort you feel in your lower abdomen and back could be related to these conditions.

Additionally, the fact that your symptoms improve when lying down and worsen after eating could suggest gastrointestinal involvement, such as irritable bowel syndrome (IBS) or other digestive issues. However, given the gynecological context of your symptoms, it is crucial to prioritize a thorough evaluation of potential reproductive health issues.

Given that you have been on hormonal birth control (悦己) for three years, it is also important to consider how this may affect your symptoms. Hormonal contraception can sometimes mask underlying conditions like endometriosis or fibroids, making it essential to communicate any changes in your symptoms to your healthcare provider.

In light of these considerations, I recommend that you seek a comprehensive evaluation from a gynecologist. They may perform a pelvic exam, ultrasound, or other diagnostic tests to determine the underlying cause of your symptoms. It is crucial to address any potential infections or reproductive health issues promptly to prevent complications.

In summary, your symptoms of right lower abdominal pain, pressure, and unusual discharge could be indicative of several conditions, including pelvic inflammatory disease, ovarian cysts, endometriosis, or gastrointestinal issues. A thorough evaluation by a healthcare professional is essential to accurately diagnose and treat your condition. Remember, early intervention can lead to better outcomes and relief from your symptoms.
