Please ask your two questions?
Hello doctor, I would like to ask about an issue I experienced.
I was lying in bed in a poor position and suddenly tried to sit up like doing a sit-up.
In that moment, I felt a sudden pulling or cramping sensation near my buttocks, close to the anus and groin area.
Although I don't feel anything now, I am wondering if this issue requires examination.
I'm concerned it might be related to some problem or sciatica.
My sister mentioned it could be due to the cold weather...
could that cause cramps? Additionally, I recently noticed that a few of my toenails appear to have a slightly deeper line at certain angles.
I'm not sure if this is the same as the black line mentioned online.
I hope the doctor can help me with this.
Thank you.

Dr. Hong Yuanbin reply Internal Medicine
Hello,
1.
What you mentioned is likely due to muscle strain caused by poor posture.
If there are no specific discomforts afterward, it should be fine to just monitor it.
2.
Regarding the nails, it appears that the variation in color is simply due to changes in blood flow and does not indicate any pathology.
Thank you for your inquiry.

More Info
When experiencing sudden muscle pain, especially in the lower back or pelvic area, it’s important to consider several factors before deciding whether to seek medical attention. Based on your description, it sounds like you may have experienced a muscle strain or spasm, which can occur when you suddenly change positions or exert yourself unexpectedly. This type of pain is often temporary and may resolve on its own, but there are some considerations to keep in mind.
Muscle Pain After Sudden Movements
1. Nature of the Pain: The sudden pain you felt near the buttocks and pelvic area could be due to a muscle strain or spasm. These types of injuries are common and can happen if the muscles are stretched beyond their normal limits. If the pain has subsided and you are not experiencing any ongoing discomfort, it may not be a cause for concern. However, if the pain persists or worsens, it would be wise to consult a healthcare professional.
2. Symptoms to Watch For: If you experience additional symptoms such as numbness, tingling, weakness in the legs, or pain radiating down the legs, it could indicate a more serious issue, such as sciatica or nerve compression. In such cases, seeking medical evaluation is advisable.
3. Environmental Factors: Your sister mentioned that cold weather might contribute to muscle cramps. While cold temperatures can lead to muscle tightness and spasms, they are not typically a direct cause of muscle strains. However, staying warm and stretching before physical activity can help prevent muscle issues.
Nail Changes
Regarding the changes in your nails, particularly the appearance of a dark line or stripe, it’s essential to approach this with caution:
1. Understanding Nail Changes: Dark lines or streaks in the nails can be caused by various factors, including trauma, fungal infections, or more serious conditions like melanoma (a type of skin cancer). If the line is new, changing, or accompanied by other symptoms (such as pain or changes in nail texture), it warrants further investigation.
2. When to Seek Medical Advice: If the dark line persists, changes in appearance, or if you have any concerns about it, it is advisable to see a dermatologist. They can perform a thorough examination and, if necessary, a biopsy to rule out any serious conditions.
Recommendations
- Monitor Symptoms: Keep an eye on your muscle pain and any changes in your nails. If the muscle pain returns or if you notice any new symptoms, consider scheduling an appointment with a healthcare provider.
- Consult a Specialist: If you have concerns about the nail changes, particularly if they are accompanied by other symptoms, consult a dermatologist for a professional evaluation.
- Self-Care: For muscle pain, consider applying heat or cold to the affected area, resting, and gently stretching to alleviate discomfort. Over-the-counter pain relievers may also help if needed.
In summary, while your muscle pain may not require immediate medical attention if it has resolved, any persistent or worsening symptoms should be evaluated by a healthcare professional. Similarly, changes in your nails should be assessed by a specialist to ensure they are not indicative of a more serious condition. Always trust your instincts—if something feels off, it’s better to err on the side of caution and seek medical advice.
