Phimosis issue
Hello doctor, I discovered three years ago that my foreskin can be completely retracted, and since then, I have been retracting it daily for cleaning.
Occasionally, I try to retract it during an erection, but I find that I can only expose about one-third of the glans.
If I pull hard, I can manage to retract it, but it is painful, and even when I push it down forcefully during DIY attempts, it tends to revert back.
If I try to retract the foreskin during erections every day, will the condition improve?

Hello: In this situation, there may be swelling that cannot be reduced, which could necessitate medical attention.
It is advisable to seek evaluation from a urologist at the hospital rather than attempting to manage it on your own.
Wishing you good health.

More Info
Phimosis is a condition where the foreskin of the penis cannot be easily retracted over the glans (the head of the penis). It is relatively common, especially in younger males, but can persist into adulthood. Your experience of being able to retract the foreskin completely after three years, but facing difficulties during erections, is not uncommon.
Daily stretching of the foreskin can indeed be beneficial in managing phimosis. The goal of stretching is to gradually increase the elasticity of the foreskin, which may allow for easier retraction over time. Here are some points to consider regarding your situation and the potential benefits of daily stretching:
1. Gentle Stretching Techniques: It is essential to approach stretching gently. Forcing the foreskin back can lead to pain, tearing, or scarring, which may worsen the condition. Instead, consider using a method where you gently pull back the foreskin without causing pain. You can do this during bathing when the skin is more pliable due to warmth and moisture.
2. Consistency is Key: Regular stretching is crucial for seeing improvement. Aim to perform this stretching exercise daily, ideally multiple times a day, to gradually increase the skin's elasticity. Over time, this may help you achieve a greater range of motion with less discomfort.
3. Moisturizing Agents: Some individuals find that applying a water-based lubricant or a topical steroid cream (as prescribed by a healthcare provider) can help soften the skin and make stretching easier. This can also reduce friction and discomfort during the process.
4. Avoid Painful Techniques: If you experience significant pain while attempting to retract the foreskin, it is advisable to stop and consult a healthcare professional. Pain can indicate that you are stretching too aggressively or that there may be other underlying issues, such as scarring or infection.
5. Consultation with a Specialist: If daily stretching does not yield improvement or if you continue to experience pain, it may be beneficial to consult a urologist. They can assess your condition and discuss other treatment options, which may include topical treatments, preputioplasty, or circumcision, depending on the severity of the phimosis and your personal preferences.
6. Psychological Aspect: It’s also important to consider the psychological aspect of this condition. Anxiety about the inability to retract the foreskin, especially during sexual arousal, can lead to stress. Open communication with partners and understanding that this is a common issue can alleviate some of that anxiety.
In summary, daily stretching can potentially improve your condition, but it should be done gently and consistently. If you encounter persistent issues or pain, seeking professional medical advice is crucial to explore other treatment options. Remember, everyone's body is different, and what works for one person may not work for another, so patience and care are essential in this process.
