Bowel issues?
Hello, doctor.
For the past six months, I have frequently experienced diarrhea in the mornings, but sometimes it turns into constipation.
I also pass gas a lot.
When I have diarrhea, my stools are very soft, but there is no blood present.
The diarrhea is particularly severe when I am at school.
I do not have any abdominal discomfort, and my weight has not changed.
I previously consulted a traditional Chinese medicine doctor, who suspected it might be irritable bowel syndrome (IBS).
I often alternate between diarrhea and constipation, which is quite distressing for me.
Doctor, could these symptoms be indicative of IBS? What methods can I use to improve my condition? Would additional tests be necessary?

It is likely irritable bowel syndrome; monitor for any blood in the stool or weight loss.
Maintain a normal diet, exercise regularly, and avoid prolonged sitting.

More Info
Irritable Bowel Syndrome (IBS) is a common gastrointestinal disorder characterized by a combination of symptoms, including abdominal pain, bloating, and alterations in bowel habits, such as diarrhea and constipation. Your symptoms of alternating diarrhea and constipation, along with excessive gas, align with the typical presentation of IBS.
The diagnosis of IBS is primarily based on clinical criteria, specifically the Rome IV criteria, which require the presence of abdominal pain at least one day per week in the last three months, associated with two or more of the following: related to defecation, a change in the frequency of stool, and a change in the form (appearance) of stool. Since you mentioned experiencing these symptoms over the past six months, it is plausible that you may be dealing with IBS.
The exact cause of IBS is not fully understood, but several factors may contribute to its development, including abnormal gut motility, visceral hypersensitivity (increased sensitivity of the intestines), and psychological factors such as stress and anxiety. Dietary factors can also play a significant role, as certain foods may trigger symptoms in some individuals.
To manage your symptoms effectively, consider the following strategies:
1. Dietary Modifications: Keeping a food diary can help identify specific foods that may trigger your symptoms. Common culprits include high-fat foods, dairy products, caffeine, and certain carbohydrates known as FODMAPs (fermentable oligosaccharides, disaccharides, monosaccharides, and polyols). A low-FODMAP diet has been shown to alleviate symptoms in many individuals with IBS.
2. Regular Meal Patterns: Eating smaller, more frequent meals rather than large meals can help regulate bowel movements and reduce symptoms.
3. Hydration: Ensure you are drinking enough water, especially if you are experiencing diarrhea. Staying hydrated is crucial for overall health and can help manage constipation.
4. Physical Activity: Regular exercise can help improve gut motility and reduce stress, which may alleviate symptoms of IBS.
5. Stress Management: Since stress can exacerbate IBS symptoms, incorporating relaxation techniques such as mindfulness, yoga, or meditation can be beneficial.
6. Medications: Depending on your symptoms, your healthcare provider may recommend medications. For diarrhea, loperamide (Imodium) can be effective, while laxatives may be prescribed for constipation. In some cases, medications specifically for IBS, such as rifaximin or eluxadoline, may be considered.
7. Probiotics: Some studies suggest that probiotics may help restore the balance of gut bacteria and improve symptoms in individuals with IBS.
8. Consultation with a Specialist: If your symptoms persist or worsen, or if you experience alarming symptoms such as weight loss, severe pain, or blood in your stool, further evaluation may be warranted. This could include tests such as a colonoscopy or imaging studies to rule out other conditions.
In summary, your symptoms are consistent with IBS, and there are several management strategies you can employ to help alleviate your discomfort. It is essential to work closely with your healthcare provider to develop a tailored plan that addresses your specific needs and symptoms. Regular follow-ups can help monitor your condition and make necessary adjustments to your treatment plan.
