Dizziness While Driving: Potential Neurological Concerns - Neurology

Feeling dizzy and unwell while driving?

Hello, doctor.
I am inquiring on behalf of my family, specifically my mother, who is 28-30 years old.
She traveled from Taipei to Yilan and Lala Mountain, driving through the Snow Tunnel and navigating over four hours of mountainous roads.
The driver experienced significant stress while driving through the Snow Tunnel, and there were issues with landslides on the way back to Taipei from Taoyuan, which added to the pressure.
Since the start of the drive, the driver has been feeling dizzy and unwell (this has not happened before during driving).
In the following days, the same symptoms persisted while driving (initially, we suspected that sleeping outside might have affected her rest).
However, my family mentioned that she felt dizzy from the very first day of driving, and at one point, we even had to switch drivers, which has never happened before.
My family plans to monitor her condition and seek medical attention if necessary, but I am very concerned and wanted to ask for your advice.
I apologize for the lengthy explanation.
I am particularly worried about potential issues related to the brain, blood pressure, or stroke.
Additionally, my mother is taking medication for blood sugar control and has a history of cancer for which she is also on medication.

Chen, 50~59 year old female. Ask Date: 2023/01/31

Dr. Jiang Junyi reply Neurology

Hello: You mentioned experiencing dizziness and discomfort...
taking medications for lowering blood sugar, and having a history of cancer with ongoing medication.
If, in addition to dizziness, there are sudden neurological deficits such as limb weakness or slurred speech, the first consideration should be cerebrovascular disease.
Your mother has significant risk factors: diabetes.
Additionally, hypertension and hyperlipidemia are concerns (I am unsure about any exposure to tobacco products).
Prolonged exposure to these risk factors can lead to atherosclerosis, increasing the likelihood of cerebrovascular and cardiovascular diseases.
If symptoms persist without improvement, I recommend consulting a neurologist for evaluation and treatment.
If there is rapid progression, it is advisable to go to the emergency room.
I suggest you and your mother visit a neurology outpatient clinic for follow-up examinations.
Thank you for your inquiry, and feel free to discuss further.
Wishing you good health and peace.
Dr.
Jiang Junyi, Taoyuan Medical Center, cares for you.

Dizziness while driving can be a concerning symptom, especially when it occurs suddenly and is accompanied by a history of stress or medical conditions. In the case of your family member, several factors could be contributing to this experience.
Firstly, it’s important to consider the context in which the dizziness occurred. Driving through mountainous terrain and under stressful conditions, such as navigating through a tunnel and dealing with potential road hazards, can lead to a heightened state of anxiety. This anxiety can manifest physically, resulting in symptoms such as dizziness, lightheadedness, or even a sense of disorientation. The stress of the situation may have triggered a physiological response, including increased heart rate and changes in blood pressure, which can contribute to feelings of dizziness.

Additionally, if your family member has been experiencing poor sleep, this could exacerbate the situation. Lack of sleep can affect cognitive function, reaction times, and overall physical health, making it more difficult to handle stressful situations. Sleep deprivation can also lead to symptoms such as dizziness, fatigue, and difficulty concentrating, which could explain the discomfort experienced while driving.

Another important consideration is the potential for underlying medical conditions. Given that your family member is on medication for blood sugar management and has a history of cancer, it’s crucial to evaluate how these factors might be influencing her current health. Some medications can have side effects that include dizziness or lightheadedness, particularly if they affect blood pressure or blood sugar levels. It would be wise to review her medications with a healthcare provider to ensure they are not contributing to her symptoms.

Moreover, neurological concerns should not be overlooked. While it’s less common, dizziness can sometimes indicate issues such as vestibular disorders, which affect balance and spatial orientation. Conditions like benign paroxysmal positional vertigo (BPPV) or vestibular neuritis could lead to dizziness, especially in situations that require quick head movements, such as driving. Additionally, while rare, dizziness can be a symptom of more serious conditions like transient ischemic attacks (TIAs) or strokes, particularly in individuals with risk factors such as hypertension or diabetes.

Given these considerations, it would be prudent for your family member to consult with a healthcare professional. A thorough evaluation, including a review of her medical history, medications, and possibly neurological assessments, can help determine the underlying cause of her dizziness. In the meantime, it may be beneficial for her to avoid driving until the cause of her symptoms is identified and managed appropriately.

In summary, dizziness while driving can stem from a combination of stress, sleep deprivation, medication effects, and potential underlying medical conditions. It’s essential to monitor her symptoms closely and seek medical advice to ensure her safety and well-being.
