Cat Bite on Hand: Persistent Pain Beyond the Wound Area - Surgery

Cat bite on the back of the hand, pain in areas not directly at the wound site?

A few days ago, I was bitten on the back of my hand (around the wrist area) by my cat at home.
I promptly went to the hospital for emergency care and received a tetanus shot.
Currently, the wound has scabbed over, and the bruising has mostly subsided, but there is still bruising at the first joint of my four fingers, and it hurts when pressed.
Additionally, there is pain in the non-injured area of the back of my hand when I make a fist or rotate my wrist, and it also hurts when I try to lift heavier objects.
I would like to know if I need to seek medical attention for this.
Would a visit to a general surgeon be sufficient? Thank you.

Hello! It is recommended to return for a follow-up visit to check for cellulitis that may be causing swelling and compressing the superficial nerves.
Take care! Changhua Hospital is concerned about you!

It sounds like you are experiencing persistent pain in your hand following a cat bite, which is concerning. Cat bites can lead to various complications, including infections, which may not always be immediately apparent. Given that you have already received a tetanus shot and your wound is healing, it is important to consider the following points regarding your symptoms and whether further medical evaluation is necessary.

1. Infection Risk: Cat bites can introduce bacteria into the skin, and even if the wound appears to be healing, there is a risk of deeper tissue infection. Symptoms of infection may include increased redness, swelling, warmth around the wound, or discharge. If you notice any of these signs, you should seek medical attention promptly.

2. Pain Beyond the Wound Area: The pain you are experiencing in the joints and surrounding areas could be due to several factors:
- Tendon or Ligament Injury: The bite may have caused trauma to the tendons or ligaments in your hand, leading to pain during movement or when pressure is applied.

- Nerve Involvement: If the bite was deep enough, it might have affected nearby nerves, causing pain that radiates beyond the initial injury site.

- Referred Pain: Sometimes, pain in one area can be felt in another due to the way nerves are interconnected.

3. Joint Pain: The pain in your finger joints and wrist could indicate inflammation or injury to the joint structures. This might be due to direct trauma from the bite or secondary to the healing process. If you are experiencing persistent pain, especially with movement, it may warrant further investigation.

4. When to Seek Medical Attention: Given that you are experiencing ongoing pain and discomfort, it would be advisable to consult with a healthcare professional. An orthopedic specialist or a hand surgeon would be appropriate, as they can assess for any potential injuries to the bones, tendons, or ligaments that may not be visible externally. They may recommend imaging studies, such as X-rays or an MRI, to evaluate the extent of any damage.

5. Management and Treatment: If an injury is confirmed, treatment may include:
- Rest and Immobilization: Keeping the hand immobilized can help reduce pain and allow for healing.

- Physical Therapy: Once the initial pain subsides, physical therapy may be beneficial to restore range of motion and strength.

- Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage pain and inflammation.

In conclusion, while it is good that your wound is healing, the persistent pain you are experiencing in areas beyond the bite site should not be ignored. It is advisable to seek further evaluation from a healthcare professional to rule out any complications and to receive appropriate treatment. Early intervention can help prevent long-term issues and ensure a full recovery.
