Pain in the lower abdomen upon exertion after a car accident?

Hello doctor, I was in a motorcycle accident on January 31, where I was hit by a car.
Besides some abrasions, I experienced significant soreness in my abdominal muscles the next day.
I initially thought it would improve in a few days, but recently, whenever I exert my lower abdominal muscles, I feel a dull pain and have difficulty bending down, lifting things, or exerting myself.
I would like to know what might be the cause of this.

yu, 20~29 year old female. Ask Date: 2023/02/12

Dr. Hong Yuanbin reply Internal Medicine

Hello, based on your description, it seems to be an abdominal muscle strain.
I recommend applying a warm compress and using some anti-inflammatory medication, which should gradually improve the condition.
If the symptoms persist, it is advisable to visit a surgical or family medicine clinic for further evaluation.
Thank you for your inquiry.

After a car accident, experiencing lower abdominal pain can be concerning and may stem from several potential causes. Given your description of the pain and the circumstances surrounding the accident, it is likely that you are dealing with a muscle strain or injury in the abdominal region.
When you were involved in the accident, the impact could have caused your abdominal muscles to stretch or tear, especially if you braced yourself or tensed your muscles upon impact. This type of injury is common in accidents where the body is subjected to sudden forces, leading to strains in the muscles that support the abdomen. The soreness you feel, particularly when engaging those muscles, suggests that they may be inflamed or healing from a strain.

In addition to muscle strain, it is essential to consider other potential injuries that could result from a car accident. For instance, internal injuries, although less common, can occur and may present as abdominal pain. These could include injuries to organs such as the spleen, liver, or intestines, especially if there was significant force involved in the collision. Symptoms of internal injuries can sometimes be subtle, so it is crucial to monitor for any additional signs such as swelling, bruising, or changes in bowel habits.

To manage your symptoms, it is advisable to start with conservative treatment. Applying heat to the affected area can help alleviate muscle tension and promote blood flow, which aids in healing. Over-the-counter anti-inflammatory medications, such as ibuprofen or naproxen, can also help reduce pain and inflammation. However, if you have any underlying health conditions or are taking other medications, it is wise to consult with a healthcare professional before starting any new medication.

If your pain persists or worsens, or if you experience any new symptoms such as fever, vomiting, or significant changes in your bowel movements, it is crucial to seek medical attention promptly. A healthcare provider may recommend imaging studies, such as an ultrasound or CT scan, to rule out any serious internal injuries.

In summary, while it is likely that your lower abdominal pain is due to a muscle strain from the accident, it is essential to remain vigilant for any signs of more severe injuries. Taking care of your body through rest, heat application, and appropriate medication can facilitate recovery. However, do not hesitate to reach out to a healthcare professional for a thorough evaluation if your symptoms do not improve or if you have any concerns about your condition.
