Anxiety Panic Disorder
Hello Dr.
Wu,
I often experience rapid heartbeat and missed beats, so I consulted a cardiologist.
The cardiologist indicated that there are no significant heart issues, and that my discomfort is mainly due to autonomic nervous system problems.
Consequently, I sought help from a psychosomatic specialist.
I tend to feel anxious about various matters and have anticipatory anxiety regarding the future, which makes me fearful.
Additionally, when my anxiety reaches its peak, I experience panic attacks characterized by rapid heartbeat, feeling faint, overwhelming fear, and concerns about dying.
I have had multiple episodes while on airplanes and high-speed trains.
After consulting with the psychosomatic specialist, the doctor diagnosed me with anxiety and panic disorder.
However, due to my immune thrombocytopenic purpura (ITP), the doctor discussed the potential bleeding risks associated with prescribing SSRIs and instead prescribed NDRI Bupropion 150 mg (one in the morning), Inderal, and Fludiazepam (one of each in the morning and evening).
I would like to ask you a few questions:
1.
Is Bupropion effective for anxiety and panic disorders?
2.
Will taking Bupropion cause my heart rate to increase or exacerbate my anxiety and panic?
3.
Are dizziness or extreme fatigue common side effects of Bupropion? I am on my fourth day of medication and have been feeling dizzy and lightheaded throughout the day, along with persistent anxiety and panic.
4.
Given that my platelet count is between 10,000 and 20,000, is it truly unsafe for me to take SSRIs? I hope to take the most effective and appropriate medication.
Thank you for taking the time to read my questions, and I would appreciate your responses.
Thank you.

Dear Patient,
Thank you for reaching out with your concerns regarding anxiety and panic disorders. It’s important to address both the psychological and physiological aspects of your symptoms, especially since they can significantly impact your quality of life. Let’s break down your questions and concerns one by one.
1. Effectiveness of Bupropion for Anxiety and Panic Disorders: Bupropion is primarily classified as a norepinephrine-dopamine reuptake inhibitor (NDRI) and is often used to treat depression and to help with smoking cessation. While it is not typically the first-line treatment for anxiety disorders, some patients may find it helpful, particularly if they also experience depressive symptoms. However, it’s crucial to note that Bupropion can sometimes exacerbate anxiety symptoms in certain individuals, particularly if they are predisposed to anxiety or panic attacks.
2. Potential for Increased Heart Rate and Anxiety: Bupropion can lead to increased heart rate in some patients, especially during the initial stages of treatment. This is due to its stimulating effects on the central nervous system. If you are already experiencing anxiety and panic attacks, this could potentially worsen your feelings of anxiety. It’s essential to monitor your symptoms closely and communicate any significant changes to your healthcare provider.
3. Side Effects of Bupropion: Common side effects of Bupropion can include dizziness, fatigue, and headaches. The sensation of feeling "head heavy" or lightheaded can be distressing, especially when you are already dealing with anxiety. If these symptoms persist or worsen, it’s important to discuss them with your doctor. They may consider adjusting your dosage or switching to a different medication that may be better tolerated.
4. Concerns Regarding SSRIs and Low Platelet Count: Your concern about taking SSRIs (selective serotonin reuptake inhibitors) given your low platelet count (ITP) is valid. SSRIs can sometimes increase the risk of bleeding, particularly in individuals with pre-existing conditions that affect platelet function. Your healthcare provider is likely weighing the risks and benefits of prescribing SSRIs in your case. If your anxiety symptoms are severe and significantly impairing your daily life, it may be worth discussing alternative treatments or closely monitoring your platelet levels while on SSRIs.
In addition to medication, I encourage you to consider incorporating non-pharmacological treatments into your care plan. Cognitive-behavioral therapy (CBT) is particularly effective for anxiety and panic disorders. It can help you develop coping strategies and address the thought patterns that contribute to your anxiety. Mindfulness practices, relaxation techniques, and regular physical activity can also be beneficial in managing anxiety symptoms.
Lastly, it’s essential to maintain open communication with your healthcare provider. If you feel that your current treatment plan is not effective or if you are experiencing troubling side effects, do not hesitate to reach out for a follow-up appointment. Adjustments to your medication or treatment approach may be necessary to find the most effective and tolerable solution for your situation.
Take care, and I wish you the best on your journey toward managing your anxiety and panic symptoms. Remember, you are not alone, and there are effective treatments available to help you regain control over your life.
Best regards.
