Pregnancy Urine Test Issues
Hello doctor, during the late stage of my pregnancy, my urine test showed positive for "leukocyte esterase," while everything else was normal.
The obstetrician mentioned that it was related to pregnancy and that it was not a concern as long as I did not have symptoms of a urinary tract infection, such as frequent urination or burning sensation.
Now that I have given birth five months ago, I recall that I really did not feel any symptoms of urethritis at that time.
I am curious if this situation was indeed influenced by the increased vaginal discharge during pregnancy, etc.? Thank you.

Dr. Huang Yuande reply Obstetrics and Gynecology
Yes, a portion of the leukocyte esterase can also be caused by an increased amount of vaginal discharge.

Positive leukocyte esterase in urine tests during pregnancy can be a concerning finding, but it is important to understand its implications and the context in which it occurs. Leukocyte esterase is an enzyme produced by white blood cells (WBCs), and its presence in urine typically indicates that there may be an infection or inflammation in the urinary tract. In pregnant women, however, the interpretation of this result can be more nuanced.
During pregnancy, hormonal changes can lead to increased vaginal discharge, which may contain leukocytes. This can sometimes result in a positive leukocyte esterase test even in the absence of a urinary tract infection (UTI). Additionally, the physiological changes that occur during pregnancy, such as increased blood volume and changes in kidney function, can also affect urine composition. Therefore, it is not uncommon for pregnant women to have a positive leukocyte esterase result without any accompanying symptoms of a UTI, such as dysuria (painful urination), frequency, or urgency.
In your case, since your doctor indicated that the positive leukocyte esterase finding was not concerning due to the absence of UTI symptoms, it is likely that the result was influenced by the physiological changes of pregnancy. It is also worth noting that the presence of leukocyte esterase does not automatically confirm an infection; it is merely a marker that warrants further investigation if accompanied by other symptoms or abnormal findings.
If you experienced no symptoms of a UTI during your pregnancy and your doctor did not recommend further testing, it is reasonable to conclude that the positive result was likely a benign finding. However, it is always advisable to maintain open communication with your healthcare provider regarding any concerns you may have about your health, especially during pregnancy.
In summary, while a positive leukocyte esterase test can indicate a potential infection, in the context of pregnancy, it may not always be clinically significant if there are no accompanying symptoms. The hormonal and physiological changes during pregnancy can lead to variations in urine composition, which may explain the positive result. If you have any lingering concerns or if you experience any new symptoms, it is always best to consult with your healthcare provider for personalized advice and management.
