White Bumps on the Penis: Causes and Concerns - Urology

White small particles

I have a cluster of small white bumps near the glans of my genitalia, with each cluster containing about several dozen bumps.
Each bump is approximately 0.03 centimeters in size, and I first noticed them during elementary school or middle school.
They are not painful, do not itch, and do not bleed or break.
I have not engaged in sexual intercourse yet.
Could this be a sexually transmitted infection? Will it be contagious or cause sexual dysfunction in the future? How should it be treated, or what kind of medication should I apply?

William, 10~19 year old female. Ask Date: 2003/11/18

Dr. Lin Wenxu reply Urology

Hello William, what you are seeing is likely what is known as "pearly penile papules," which are not a sexually transmitted infection.
There is no risk of transmission or sexual dysfunction associated with them, so you need not worry too much.
However, if you still have concerns, you may want to visit a urology clinic for an examination.

Understanding white bumps on the penis, particularly around the glans, can be a source of concern for many individuals. Based on your description, it sounds like you may be observing a condition known as pearly penile papules (PPP). These are small, white or skin-colored bumps that typically appear around the corona of the glans penis. They are completely normal anatomical variations and are not associated with any disease or infection.

Pearly penile papules are harmless and do not require treatment. They are not contagious and do not pose any risk of transmission during sexual activity. Importantly, they do not affect sexual function or cause any pain or discomfort, which aligns with your experience of having these bumps without any associated symptoms.

Another possibility could be Fordyce spots, which are small, painless, pale bumps that can appear on the shaft of the penis and are caused by enlarged sebaceous (oil) glands. Like pearly penile papules, Fordyce spots are also benign and do not require treatment.

It’s essential to differentiate these benign conditions from sexually transmitted infections (STIs) or other medical concerns. STIs can present with various symptoms, including sores, bumps, or changes in the skin, often accompanied by pain, itching, or discharge. Since you mentioned that the bumps are not painful, itchy, or bleeding, it is less likely that they are related to an STI.

If you are still concerned about the bumps or if you notice any changes in their appearance, such as an increase in size, color change, or the development of new symptoms, it would be advisable to consult a healthcare professional, such as a dermatologist or urologist. They can provide a thorough examination and, if necessary, perform tests to rule out any other conditions.

In terms of treatment, if the bumps are indeed pearly penile papules or Fordyce spots, no treatment is necessary. However, if you are looking for cosmetic removal, options such as laser therapy or cryotherapy can be discussed with a qualified healthcare provider. It’s crucial to avoid self-treatment or using over-the-counter medications without professional guidance, as this could lead to irritation or other complications.

In summary, the white bumps you are observing are most likely benign and not indicative of a sexually transmitted infection or any serious health concern. They are common anatomical variations that many men have. However, if you have any doubts or if your situation changes, seeking medical advice is always a prudent approach. Remember, maintaining open communication with healthcare providers about any concerns regarding your sexual health is vital for overall well-being.
