Vulvar itching with a white, flaky layer on the vulva?
Hello Doctor: For the past two months, I have been suffering from persistent itching in the vulvar area, to the point where it is unbearable.
Sometimes it feels like something is crawling, and most of the time it is extremely itchy, and scratching does not provide any relief.
I have also noticed a white layer on the vulvar skin, which flakes off when I scratch, but I haven't observed any unusual discharge.
I have already visited a gynecology clinic, and they diagnosed it as lichen sclerosus, attributing it to an inability to keep the area dry.
They prescribed an ointment, but it seems to make the itching worse, and it feels very much like something is crawling.
Could it be possible that I have an infection as well? Thank you, Doctor!

More Info
Vulvar itching and the presence of white flaky skin can be distressing symptoms that may arise from various underlying conditions. Understanding the potential causes and appropriate solutions is essential for effective management.
Possible Causes
1. Lichen Sclerosus: This is a chronic inflammatory skin condition that often affects the vulvar area, leading to white patches, itching, and discomfort. It can cause thinning of the skin and may result in scarring if left untreated.
2. Yeast Infections: A common cause of vulvar itching, yeast infections can lead to white, flaky skin and a thick, white discharge. While you mentioned that there were no unusual secretions, it’s still possible for a mild infection to cause localized symptoms without significant discharge.
3. Contact Dermatitis: This occurs when the vulvar skin reacts to irritants or allergens, such as soaps, detergents, or personal hygiene products. Symptoms include itching, redness, and flaking skin.
4. Bacterial Infections: Certain bacterial infections can also cause itching and irritation in the vulvar area. These may not always present with noticeable discharge but can still lead to discomfort.
5. Hormonal Changes: Fluctuations in hormone levels, particularly during menopause, can lead to vulvar dryness and irritation, which may manifest as itching and flaky skin.
6. Psoriasis or Eczema: These skin conditions can affect any part of the body, including the vulvar area, leading to itching, redness, and scaling.
Solutions and Management
1. Consultation with a Specialist: Since you have already visited a gynecologist, it may be beneficial to seek a second opinion or consult a dermatologist who specializes in skin conditions. They can provide a more comprehensive evaluation and possibly perform a biopsy if necessary.
2. Topical Treatments: If lichen sclerosus is confirmed, topical corticosteroids are often prescribed to reduce inflammation and itching. However, if the prescribed ointment exacerbates your symptoms, it’s crucial to communicate this with your healthcare provider. They may adjust the treatment plan or suggest alternative medications.
3. Avoid Irritants: Identify and eliminate any potential irritants from your hygiene routine. Use gentle, fragrance-free products for washing and avoid douching or using scented feminine products.
4. Moisturizers: Regularly applying a hypoallergenic moisturizer can help maintain skin hydration and reduce itching. Look for products specifically designed for sensitive skin.
5. Antihistamines: Over-the-counter antihistamines may help alleviate itching, especially if it is related to an allergic reaction.
6. Lifestyle Modifications: Wearing breathable cotton underwear and avoiding tight clothing can help keep the area dry and reduce irritation. Additionally, maintaining good hygiene while ensuring the area is not overly dry is crucial.
7. Diet and Hydration: Staying hydrated and maintaining a balanced diet can support overall skin health. Some individuals find that reducing sugar and processed foods can help with yeast-related issues.
Conclusion
In summary, vulvar itching and white flaky skin can stem from various causes, including inflammatory conditions, infections, and irritants. It is essential to work closely with healthcare providers to identify the underlying issue and develop an effective treatment plan. If your symptoms persist or worsen, do not hesitate to seek further medical advice. Your comfort and health are paramount, and there are solutions available to help alleviate your symptoms.
