Testicular issues and penile issues?


I have a severe case of gynecomastia, to the point where my nipples are inverted.
I am not very willing to discuss this with my family, so I have not sought medical examination.
The only times I have had check-ups were during my middle school, high school, and university health screenings, where the reports indicated no issues with my testicles, which reassured me.
My penis measures three centimeters when not erect, but can reach ten centimeters when erect.
During middle and high school, I noticed significant fluctuations in the contraction of my scrotum, but I didn't pay much attention to it; I was mainly troubled by the length of my penis for a long time.

In my senior year of high school, about two years ago, I experienced severe pain above my testicles four or five times.
Each episode lasted about two to three minutes, and the pain was so intense that I could hardly stand up from my chair.
At that time, I didn't want to take time off for a doctor's visit due to my exams, and I also didn't want my family to know, so I didn't seek medical attention.
After that, I didn't experience any pain again and treated it as if it never happened.
Recently, I have noticed that my scrotum is almost always in a contracted state, regardless of whether the weather is cold or hot.
Even when I switched to looser underwear for ventilation, the situation remained the same.
At its loosest, my testicles just fit inside the scrotum with no space between them, while at its tightest, they retract into my body, and this latter condition lasts for a longer duration.
Additionally, I have also noticed a retraction of my penis, which has decreased to two centimeters when not erect.
