Post-hemorrhoid surgery
Hello, doctor.
I would like to ask you about my laser minimally invasive surgery that I had in April of last year for internal and external hemorrhoids.
During this time, I have occasionally experienced slight bleeding when wiping after a bowel movement.
I had one episode of anal fissure, which I treated with medication and topical ointment.
Last Friday, I experienced severe pain during a bowel movement, and there was a small amount of blood on the toilet paper.
Since then, I have been feeling very uncomfortable with itching and irritation.
I applied hemorrhoid cream, but it hasn't improved.
Starting yesterday, I have noticed a continuous discharge of a slightly yellow fluid from my anus.
Why is this happening? I have an appointment scheduled for tomorrow, but I am very anxious, so I wanted to ask for your advice.
Thank you.

If there is pain during or after bowel movements, it is mostly related to anal fissures.
Any injury can cause slight bleeding and tissue fluid, but it does not necessarily indicate an infection or inflammation.
It is recommended to take warm sitz baths and use ointments, as well as to increase fluid intake to aid in bowel movements.
Describe your main symptoms to your doctor to assist in diagnosis, and there is no need to worry excessively.

After undergoing laser treatment for hemorrhoids, it's not uncommon to experience a range of symptoms as your body heals. Your concerns about post-surgery symptoms, such as bleeding, pain, and discharge, are valid and warrant careful consideration.
1. Post-Surgery Bleeding: It's not unusual to have some bleeding after hemorrhoid surgery, especially during bowel movements. The presence of blood on toilet paper can indicate irritation or minor trauma to the surgical site. However, if the bleeding is persistent or increases in volume, it may be a sign of complications such as infection or inadequate healing.
2. Pain and Discomfort: Experiencing pain during bowel movements is also a common symptom post-surgery. This can be due to several factors, including the healing process of the surgical site, the formation of scar tissue, or even the presence of anal fissures. If the pain is severe or accompanied by significant bleeding, it is essential to consult your healthcare provider.
3. Discharge: The yellowish discharge you mentioned could be a sign of infection or simply a part of the healing process. After surgery, it's normal for the body to produce some fluid as part of the inflammatory response. However, if the discharge has a foul odor or is accompanied by increased pain, fever, or other systemic symptoms, it could indicate an infection that requires medical attention.
4. Itching and Irritation: The itching and discomfort you are experiencing could be due to several reasons, including irritation from stool, the healing process, or even a reaction to topical treatments. It's essential to keep the area clean and dry, and avoid any irritants that may exacerbate the symptoms.
5. Follow-Up Care: Since you have a follow-up appointment scheduled, it’s crucial to discuss all of these symptoms with your healthcare provider. They can perform a thorough examination to determine the cause of your symptoms and recommend appropriate treatment. This may include medications to manage pain, topical treatments for irritation, or antibiotics if an infection is suspected.
6. Self-Care Measures: In the meantime, consider implementing some self-care measures:
- Diet: Ensure you are consuming a high-fiber diet to promote softer stools and reduce straining during bowel movements. Staying hydrated is also essential.
- Sitz Baths: Taking warm sitz baths can help soothe the area and promote healing.
- Avoid Straining: Try not to strain during bowel movements, as this can exacerbate pain and bleeding.
- Topical Treatments: If you are using any topical treatments, ensure they are suitable for post-surgical care and do not contain irritants.
7. When to Seek Immediate Care: If you experience severe pain, heavy bleeding, or signs of infection (such as fever or increasing redness and swelling), seek immediate medical attention.
In summary, while some discomfort and minor bleeding can be expected after hemorrhoid laser treatment, persistent symptoms warrant further evaluation. Your upcoming appointment will be an excellent opportunity to address these concerns and receive tailored advice for your recovery. Remember, open communication with your healthcare provider is key to managing your symptoms effectively.
