Anxiety and Panic
Hello Dr.
Wu, I stopped taking Prozac because I had been on it for over a year and my condition was stable, so I gradually tapered off.
Now I am experiencing a slight relapse.
The doctor prescribed Yongkangxu, to be taken once in the morning.
I would like to ask Dr.
Wu about an incident that happened last night.
After taking a shower, I was lying in bed playing on my phone when I suddenly felt very cold, to the point of shivering.
I became very anxious and measured my blood pressure, which was 187/87/115.
Later, while lying in bed, I continued to feel very cold and panicky, and I really wanted to go to the emergency room.
After taking one Xanax, my blood pressure gradually decreased to 150, then 140, and then 130.
Does this count as a panic attack? I am really worried it might be a heart issue, but I have had tests done before.
Please help me, thank you Dr.
Wu.

More Info
Panic attacks can be a distressing experience, often characterized by sudden and intense feelings of fear or discomfort that peak within minutes. Understanding the symptoms, causes, and treatment options is crucial for managing this condition effectively.
Symptoms of Panic Attacks
The symptoms of panic attacks can vary from person to person, but they typically include:
1. Physical Symptoms: These can include heart palpitations, sweating, trembling, shortness of breath, feelings of choking, chest pain, nausea, dizziness, chills, or hot flashes. In your case, the sudden coldness and shivering after a shower, followed by elevated blood pressure, could indicate a panic response.
2. Psychological Symptoms: Many individuals report feelings of unreality, fear of losing control, or fear of dying during an attack. The intense anxiety can lead to a sense of impending doom.
3. Behavioral Symptoms: After experiencing a panic attack, individuals may start to avoid situations where they fear an attack might occur, leading to a cycle of avoidance behavior.
Causes of Panic Attacks
Panic attacks can be triggered by various factors, including:
- Genetics: A family history of anxiety disorders can increase the likelihood of experiencing panic attacks.
- Stress: Major life transitions, trauma, or chronic stress can trigger panic attacks.
- Biological Factors: Imbalances in neurotransmitters, particularly serotonin and norepinephrine, may play a role.
- Environmental Factors: Certain situations, such as crowded places or stressful work environments, can act as triggers.
Treatment Options
1. Medication: Your current prescription of Yongkang (likely a form of an SSRI) is a common treatment for anxiety and panic disorders. SSRIs like Prozac (fluoxetine) help regulate mood and reduce anxiety over time. It’s essential to follow your doctor’s instructions regarding dosage and duration. If you experience a resurgence of symptoms, it may be necessary to reassess your medication regimen.
2. Cognitive Behavioral Therapy (CBT): This form of therapy is highly effective for panic disorder. It helps individuals identify and change negative thought patterns and behaviors associated with panic attacks.
3. Lifestyle Modifications: Regular physical activity, a balanced diet, and adequate sleep can significantly impact anxiety levels. Mindfulness practices, such as meditation and deep-breathing exercises, can also help manage symptoms.
4. Emergency Strategies: During a panic attack, techniques such as grounding exercises (focusing on your surroundings) or controlled breathing can help reduce symptoms. Carrying a fast-acting medication like Xanax (alprazolam) can also be beneficial for acute episodes.
Conclusion
Based on your description, it seems that the symptoms you experienced after showering—feeling cold, trembling, and elevated blood pressure—could indeed be indicative of a panic attack, especially if they were accompanied by feelings of anxiety and fear. It’s understandable to be concerned about potential heart issues, especially given your history. However, since you have previously undergone evaluations without findings of heart problems, it may be more likely that these symptoms are related to anxiety rather than a cardiac event.
It’s crucial to maintain open communication with your healthcare provider about your symptoms and any concerns you have. If your symptoms persist or worsen, or if you feel overwhelmed, seeking immediate medical attention is always a wise choice. Remember, managing panic attacks often requires a multifaceted approach, combining medication, therapy, and lifestyle changes.
