After embryo implantation failure

Hello doctor, my wife had two embryos of average quality implanted, but a few weeks later it was declared unsuccessful.
Two days after her menstrual cycle, she noticed a semen smell in her underwear.
Is this phenomenon common? Is it normal?

Hello: If there is an unusual odor or a fishy smell from the vagina, or if there is an increase in discharge accompanied by discomfort, it may indicate a vaginal infection or inflammation.
Please visit a gynecologist for an examination.

After an embryo transfer, many patients experience a range of symptoms as their bodies adjust to the hormonal changes and the implantation process. It's important to understand that these symptoms can vary widely from person to person. In your case, the presence of a semen odor in your wife's underwear a couple of days after her menstrual cycle could be concerning, but it may not necessarily indicate a problem.

Firstly, it's essential to recognize that after an embryo transfer, the body is subjected to various hormonal treatments, including progesterone, which is often administered to support the uterine lining and facilitate implantation. These hormonal changes can lead to increased vaginal discharge, which may have a different consistency or odor compared to what is typically experienced.
The odor you are describing could be attributed to several factors:
1. Residual Semen: If the embryo transfer was performed using fresh sperm, it's possible that some residual semen could remain in the vaginal canal. Semen can have a distinct odor, and if it hasn't been fully expelled, it may be noticeable in the days following the procedure.

2. Hormonal Changes: The hormonal medications used during the IVF process can alter the vaginal flora and pH balance, potentially leading to changes in discharge and odor. This is usually not a cause for concern unless accompanied by other symptoms such as itching, burning, or unusual color.

3. Infection: While less likely, it's important to consider the possibility of an infection, especially if the odor is strong or accompanied by other symptoms. Infections can occur in the reproductive tract and may require medical evaluation and treatment.

4. Menstrual Cycle: Since you mentioned that your wife noticed this odor shortly after her menstrual cycle, it’s also possible that the odor is related to the normal changes that occur during menstruation. Blood and tissue can have a distinct smell, and this may be more pronounced if there are remnants of menstrual flow.

In general, if the odor is mild and not accompanied by any other concerning symptoms, it may be considered normal. However, if your wife experiences any discomfort, unusual discharge, or if the odor becomes particularly strong, it would be advisable to consult with her healthcare provider. They can perform an examination and, if necessary, run tests to rule out any infections or other issues.

In conclusion, while the presence of a semen odor after an embryo transfer can be concerning, it is not uncommon and may be attributed to several benign factors. Monitoring for any additional symptoms and maintaining open communication with her healthcare provider will be key in ensuring her health and addressing any concerns that may arise during this sensitive time.
