Abnormal bleeding

Hello, doctor.
I had my menstrual period starting on March 2, which lasted until around March 9 (generally occurring every 28 to 31 days and lasting about 7 to 8 days).
After it ended, I began drinking Si Wu Decoction and soaking Astragalus tablets in water.
On March 15, I started to notice a small amount of bright red blood, and on March 16, the amount increased, resembling the start of my menstrual period again.
I experienced mild discomfort in my lower abdomen, but it didn't last long.
I would like to know what might be causing this situation, as this is the first time I have experienced something like this, and I am quite worried and anxious.
Thank you.

Ling Xiaojie, 30~39 year old female. Ask Date: 2023/03/16

Abnormal bleeding can be a source of significant concern for many women, and understanding its potential causes is crucial for managing health and alleviating anxiety. Based on your description, it appears that you experienced a normal menstrual cycle followed by unexpected bleeding shortly after your period ended. This situation can arise from various factors, and I will outline some possible causes and considerations.

Firstly, the timing of your bleeding is noteworthy. You mentioned that your menstrual cycle typically lasts between 28 to 31 days, with periods lasting about 7 to 8 days. After your period ended on March 9, you began experiencing light red bleeding on March 15, which then increased in volume, resembling a menstrual flow. This pattern could suggest a few possibilities:
1. Ovulation Bleeding: Some women experience light spotting around the time of ovulation, which typically occurs about 14 days before the next expected period. This is known as ovulation bleeding and is caused by a temporary drop in estrogen levels that can lead to slight shedding of the uterine lining. If your cycle is regular, this could be a plausible explanation for the bleeding.

2. Hormonal Fluctuations: The use of herbal remedies such as "四物飲" (Four Substance Decoction) and "黃耆" (Astragalus) may influence hormonal levels in your body. While these remedies are often used to promote health and balance, they can also lead to unexpected changes in menstrual patterns, especially if they affect estrogen or progesterone levels.

3. Endometrial Changes: If there are changes in the endometrial lining, such as thickening or polyps, this could lead to abnormal bleeding. However, since you mentioned that this is the first occurrence of such bleeding, it may be less likely to be a structural issue, especially if you have had recent gynecological evaluations that ruled out significant abnormalities.

4. Stress and Lifestyle Factors: Stress can have a profound impact on menstrual cycles. Emotional stress, changes in routine, or even physical stress from exercise can lead to irregular bleeding. If you have experienced any significant stressors recently, this could be a contributing factor.

5. Pregnancy-Related Issues: Although you did not mention any recent sexual activity, it is essential to consider the possibility of pregnancy-related bleeding, such as implantation bleeding. If there is any chance of pregnancy, a home pregnancy test could provide clarity.

6. Infection or Inflammation: In some cases, infections or inflammatory conditions of the reproductive tract can lead to abnormal bleeding. If accompanied by other symptoms such as unusual discharge, fever, or pelvic pain, it would be wise to consult a healthcare provider.

Given your concerns and the recent onset of this bleeding, it is advisable to monitor the situation closely. If the bleeding persists, increases in volume, or is accompanied by severe pain or other concerning symptoms, seeking medical attention is crucial. A healthcare provider may recommend further evaluation, including a pelvic exam, ultrasound, or hormonal assessments, to determine the underlying cause of your abnormal bleeding.

In conclusion, while occasional abnormal bleeding can be common and often benign, it is essential to pay attention to your body and seek medical advice when necessary. Understanding the potential causes can help alleviate anxiety and guide you toward appropriate management strategies. Remember, your health and well-being are paramount, and proactive communication with your healthcare provider is key to addressing any concerns you may have.
