Spotting and Fibroids While on Birth Control Pills - Obstetrics and Gynecology

Oral contraceptives

Hello: I have been taking oral contraceptives (Yueji) for over two years and have consistently remembered to take them.
Occasionally, I experience spotting before my period, but the gynecologist has confirmed that everything is normal.
Last week, I needed to take two packs of Yueji consecutively to delay my period for a trip abroad.
In the first few days of the second pack, I also had some spotting, which then stopped.
I am currently continuing to take the pills.
Last week, during a health check, the ultrasound technician mentioned a possible fibroid (or cyst, she was unsure) and advised me to follow up with a gynecologist.
A few days after that check, I started experiencing spotting again.
Since the report will take two weeks to come back, I am uncertain if my current situation is normal.
Should I wait for the report before seeking further medical evaluation, or is it advisable to see a doctor sooner? Additionally, given my long-term use of contraceptives, is there a possibility of pregnancy in this situation? Thank you very much for your response!

Hello, the examination is not urgent; you can wait for the report to come out before going.
You should not be pregnant.
Wishing you good health.

When it comes to understanding spotting and fibroids while on birth control pills, it’s essential to consider several factors that could be influencing your symptoms. First, let’s address the spotting you’ve experienced. Spotting, or light bleeding between periods, can be a common side effect of hormonal contraceptives, especially during the first few months of use or when there are changes in the regimen, such as taking two packs back-to-back to delay your period. This is often due to the hormonal fluctuations that occur as your body adjusts to the medication.

In your case, since you have been on birth control pills for over two years and have been consistent with your regimen, the spotting could be attributed to the recent change in your pill-taking schedule. It’s not unusual for some women to experience breakthrough bleeding when they extend their pill packs or when they first start a new pack. However, if the spotting persists or becomes heavier, it’s important to consult your healthcare provider.

Regarding the ultrasound that indicated a possible fibroid or cyst, it’s crucial to follow up with your gynecologist for further evaluation. Fibroids are non-cancerous growths of the uterus that can cause various symptoms, including heavy menstrual bleeding, pelvic pain, and sometimes spotting. While many women with fibroids do not experience significant issues, it’s essential to monitor them, especially if they are causing symptoms. Cysts, on the other hand, are fluid-filled sacs that can also occur in the ovaries and may resolve on their own without treatment.

As for your concern about whether you should wait for the report to come back before seeking further medical advice, it’s generally advisable to reach out to your healthcare provider sooner rather than later if you have ongoing symptoms like spotting. They can provide guidance on whether further investigation is needed or if your current symptoms are typical for your situation.

Regarding the possibility of pregnancy while on birth control pills, it is important to note that when taken correctly, birth control pills are highly effective at preventing pregnancy. However, no contraceptive method is 100% effective. If you have taken your pills consistently and correctly, the likelihood of pregnancy is low, but if you experience symptoms such as missed periods, significant changes in your body, or unusual symptoms, it may be worth taking a pregnancy test or consulting your healthcare provider for further evaluation.

In summary, while spotting can be a common occurrence with hormonal contraceptives, it’s essential to monitor your symptoms and consult your healthcare provider, especially in light of the ultrasound findings. They can provide personalized advice based on your medical history and current situation. Remember, your health and peace of mind are paramount, so don’t hesitate to seek medical advice if you feel uncertain or concerned about your symptoms.
