Red Sores Under the Foreskin: Balanitis or Syphilis? - Urology

There is a wound when the foreskin is retracted?

There are 4-5 red sores at the opening of the retracted foreskin, and there is a stinging sensation during urination.
There is also a red spot on the glans.
I would like to know if this is balanitis or syphilis.

When it comes to red sores under the foreskin, the differential diagnosis can include conditions such as balanitis, which is an inflammation of the glans (the head of the penis), and sexually transmitted infections (STIs) like syphilis. Given your symptoms of red sores and pain during urination, it is important to consider both possibilities.

Balanitis is commonly caused by irritation, poor hygiene, or infections (bacterial or fungal). Symptoms typically include redness, swelling, and soreness of the glans and foreskin, sometimes accompanied by discharge or a foul odor. It can occur in men of all ages but is particularly common in uncircumcised males. Treatment usually involves improved hygiene, topical antifungal or antibacterial medications, and in some cases, corticosteroids to reduce inflammation.

On the other hand, syphilis is a sexually transmitted infection caused by the bacterium Treponema pallidum. The primary stage of syphilis often presents with a painless sore (chancre) at the site of infection, which can occur on the genital area, including under the foreskin. If left untreated, syphilis can progress to secondary syphilis, which may present with rashes and other systemic symptoms. It is crucial to note that syphilis can be asymptomatic in its early stages, making it important to consider testing if there is any risk of exposure.

Given your symptoms, it is essential to seek medical attention for a proper diagnosis. A healthcare provider will likely perform a physical examination and may recommend laboratory tests, including swabs of the sores, blood tests for syphilis, and possibly urinalysis to rule out urinary tract infections (UTIs) or other conditions.

In the meantime, here are some general recommendations:
1. Maintain Hygiene: Gently clean the area with mild soap and water, ensuring to dry it thoroughly afterward. Avoid using harsh soaps or irritants that could exacerbate the condition.

2. Avoid Irritants: Refrain from using scented products, lotions, or creams on the genital area until you have a clear diagnosis.

3. Avoid Sexual Activity: Until you have been evaluated by a healthcare provider, it is advisable to abstain from sexual activity to prevent potential transmission of STIs and to avoid further irritation.

4. Monitor Symptoms: Keep track of any changes in your symptoms, such as the appearance of new sores, changes in pain levels, or any systemic symptoms like fever or malaise.

5. Seek Medical Attention: Given the potential seriousness of STIs, it is crucial to consult a healthcare professional as soon as possible for an accurate diagnosis and appropriate treatment.

In conclusion, while both balanitis and syphilis can present with similar symptoms, they require different approaches to treatment. A healthcare provider will be able to guide you through the necessary steps to address your symptoms effectively. Remember, early diagnosis and treatment are key to managing any condition effectively.
