Prepuce and glans?

Hello, Doctor.
I would like to ask:
1.
The foreskin retracts automatically during an erection, but I feel a slight stinging sensation during the retraction process (it seems to be less noticeable in summer).
When not erect, I can manually retract it, but it feels very tight against the glans (similar to peeling off the protective film from a new screen).
When fully erect, the foreskin near the corona feels very tight.
Is there a need for circumcision in this case?
2.
I usually keep the glans covered by the foreskin, but recently, over the past year, I have noticed that there is almost always a layer of smegma on the foreskin and glans after showering.
If rinsing with water or gently rubbing does not completely remove the smegma, what should I do? Would exposing the glans regularly help improve the smegma situation?
3.
I have seen other doctors recommend applying petroleum jelly after showering.
Should I use a cream or a lotion? Can a water-based lubricant used during intimate contact be a substitute? Should it be applied to both the foreskin and glans while keeping the glans exposed, or should it be applied to the glans with the foreskin covering it? Thank you, Doctor.

Hello: 1.
If you continue to experience this condition despite daily cleaning, drying, and repositioning, surgery may be necessary.
2.
After daily cleaning, drying, and repositioning, if it remains very wet after urination, you should also dry it thoroughly; avoid using soap or body wash for now.
3.
Personally, I do not recommend using petroleum jelly or lotion; just keep the area dry and reposition it.
If there is any redness, you should seek medical attention.
Wishing you good health.

Understanding foreskin and glans health is essential for maintaining genital hygiene and comfort. Let's address your concerns one by one.

1. Tightness and Pain During Erection: It’s not uncommon for some men to experience tightness or discomfort when retracting the foreskin, especially if it feels like it’s sticking to the glans (the head of the penis). This can be a sign of phimosis, a condition where the foreskin cannot be easily retracted over the glans. If you experience pain during this process, especially if it’s accompanied by any tearing or bleeding, it may be worth consulting a urologist. They can assess whether a circumcision or other surgical intervention is necessary. In some cases, gentle stretching exercises or topical treatments can help alleviate the tightness without the need for surgery.

2. Buildup of Smegma: The presence of smegma, a combination of dead skin cells, oils, and moisture, is normal, especially if the foreskin covers the glans. However, if you notice an excessive buildup that is difficult to clean, it’s important to maintain proper hygiene. Washing with warm water daily should suffice, but if you find that this isn’t effective, consider using a mild soap. Avoid harsh soaps that can irritate the sensitive skin of the glans. Keeping the glans exposed can help reduce smegma buildup, as it allows for better air circulation and less moisture retention. However, if you experience discomfort or irritation from keeping the glans exposed, it’s best to consult a healthcare professional for personalized advice.

3. Use of Vaseline and Lubricants: Applying a thin layer of Vaseline or a similar ointment can help keep the skin moisturized and reduce friction, especially if you experience dryness or irritation. It’s generally recommended to use a water-based lubricant during intimate activities, as these are designed to reduce friction and are safe for use with condoms. If you choose to apply lubricant, it’s best to apply it directly to the glans and the inner foreskin if you are retracting it. If you prefer to keep the foreskin over the glans, you can apply the lubricant to the glans and then cover it with the foreskin. This can help prevent irritation during sexual activity.

In summary, if you are experiencing pain or significant discomfort, it’s advisable to seek medical advice. A healthcare professional can provide a thorough examination and recommend appropriate treatments or interventions. Maintaining good hygiene practices, such as regular washing and using suitable moisturizers or lubricants, can significantly improve comfort and health. If you have further questions or concerns, don’t hesitate to reach out to a urologist or healthcare provider who specializes in men's health.
