Enlarged Lymph Nodes Near the Ears: Causes and Concerns - Oncology

Bilateral enlargement of the lymph nodes located beneath the ears?

Hello Doctor: Recently, I noticed swollen lymph nodes on both sides under my ears.
The left side is smaller, about 1 cm, while the right side is larger, nearly 2 cm.
They feel firm and immobile, and I do not have any symptoms of a cold, fever, or weight loss.
I visited a large hospital and consulted an ENT specialist.
I would like to ask: 1.
The doctor examined the right side with an endoscope and estimated it to be about 2 cm, indicating an 80% chance of being benign.
However, they arranged for a fine needle aspiration biopsy under ultrasound guidance but did not order any blood tests.
Is this still considered dangerous? 2.
Can lymph nodes become larger if I keep pressing on them? I noticed that when I am anxious and keep touching them, they seem to get bigger.
3.
I would like to know the possible causes of lymph node enlargement.
Could it be related to the inflammation of my jaw joints caused by my left wisdom tooth extraction in January? This has made me worry about lymphoma, as I have health anxiety, and it's hard not to focus on it.
I would appreciate your help in clarifying these concerns.
Thank you.

1.
Essentially, fine needle aspiration can obtain tissue and is the standard diagnostic method for lymph node diseases, with the pathology report being the definitive reference.

2.
Whether the lymph node enlarges with pressure depends on the force applied; normal pressure should not affect the size of the lymph nodes.

3.
Infection is also a cause of lymphadenopathy.
It is advisable to observe for two weeks, and if there is no reduction in size, it is safer to seek further evaluation at a hospital.

More Info

Enlarged lymph nodes, particularly those located near the ears, can be concerning for many individuals, especially when they are hard and immobile. In your case, you have noticed enlarged lymph nodes on both sides of your neck, with the right side being notably larger at approximately 2 cm. You have also mentioned that you do not have any accompanying symptoms such as fever, weight loss, or respiratory issues, which is a positive sign.
1. Assessment of Risk: The fact that your doctor performed an endoscopic examination and deemed the lymph node likely benign is reassuring. The recommendation for ultrasound-guided fine needle aspiration (FNA) is a common practice to further evaluate the nature of the lymph node. While any procedure carries some risk, the likelihood of serious complications from an FNA is low. The procedure is typically performed to rule out malignancies or infections and can provide valuable information about the lymph node's characteristics.

2. Impact of Manipulation: It is understandable to be concerned about the size of the lymph nodes, especially if you find yourself frequently touching or pressing on them. While it is a common belief that manipulating lymph nodes can cause them to enlarge, the actual increase in size is more likely due to underlying conditions rather than the act of touching them. However, excessive manipulation can lead to irritation or inflammation, which might contribute to a perception of increased size. It is advisable to avoid constant pressure on the lymph nodes to prevent any unnecessary irritation.

3. Potential Causes: The enlargement of lymph nodes can be attributed to various factors, including infections, inflammation, or malignancies. In your case, you mentioned a history of wisdom tooth extraction that may have caused inflammation in the jaw joint (temporomandibular joint, TMJ). Infections or inflammation in the oral cavity, including dental issues, can lead to reactive lymphadenopathy, where lymph nodes enlarge in response to nearby infections. This could explain the lymph node enlargement you are experiencing.

4. Anxiety and Health Concerns: It is important to address your anxiety regarding potential serious conditions such as lymphoma. While it is natural to seek information online, it can often lead to increased worry, especially when searching for symptoms related to serious diseases. It is crucial to focus on the medical advice provided by your healthcare professionals, who can assess your condition based on clinical findings rather than online information.
In conclusion, while the presence of enlarged lymph nodes can be concerning, your current symptoms and the findings from your medical evaluations suggest a lower likelihood of serious conditions. The ultrasound-guided FNA will provide further clarity. In the meantime, try to manage your anxiety through relaxation techniques and avoid excessive manipulation of the lymph nodes. If you notice any new symptoms or changes, do not hesitate to follow up with your healthcare provider. Your health and peace of mind are paramount, and addressing your concerns with a professional is the best course of action.
