Prostate issues
Hello doctor, I have a question.
I have phimosis, and when I go to the bathroom, I need to retract the foreskin and pull out my penis.
After that, I have to wait for about 2 to 3 seconds before I can urinate.
During urination, the flow is continuous, but after finishing, I notice a few drops of urine that I need to push out.
Could this be a prostate issue?

Hello: Generally, young people rarely experience issues with prostate enlargement; it is more often related to the bladder.
It is advisable to seek medical examination at a hospital.
Wishing you good health.

More Info
Understanding prostate issues can be quite complex, especially when symptoms overlap with other conditions. Based on your description, it seems you are experiencing a few specific urinary symptoms that may or may not be related to prostate health. Let's break down your concerns and provide some insights.
Symptoms Overview
1. Phimosis (包皮過長): You mentioned having a long foreskin, which can complicate urination. When the foreskin is not retracted properly, it may cause a delay in urination as you described—having to wait a few seconds after exposing the penis before you can urinate. This is not uncommon in individuals with phimosis, and it can lead to discomfort or difficulty in maintaining a steady urine stream.
2. Post-void dribbling: The need to exert extra effort to expel a few drops of urine after finishing urination can be indicative of several issues. This can occur due to incomplete bladder emptying, which may be related to prostate health, or it could simply be a result of pelvic floor muscle control.
3. Prostate Concerns: While your symptoms could suggest potential prostate issues, such as benign prostatic hyperplasia (BPH) or prostatitis, they are not definitive indicators. BPH typically presents with symptoms like frequent urination, urgency, weak urine stream, and difficulty starting urination. Prostatitis can cause similar urinary symptoms along with pelvic pain.
Potential Causes
- Prostate Enlargement: As men age, the prostate can enlarge, leading to urinary symptoms. However, this is more common in older adults, typically starting around age 50 and increasing in prevalence with age.
- Infection or Inflammation: Prostatitis can occur at any age and may cause urinary difficulties, pain, and discomfort.
- Pelvic Floor Dysfunction: Sometimes, the muscles that control urination can become tense or weak, leading to issues like post-void dribbling.
Recommendations
1. Consult a Urologist: Given your symptoms, it would be wise to consult a urologist. They can perform a thorough evaluation, including a physical examination, urinalysis, and possibly imaging studies or urodynamic tests to assess bladder function.
2. Consider Circumcision: If phimosis is causing significant issues, circumcision may be a solution. This procedure can alleviate the complications associated with long foreskin and may improve your overall urinary experience.
3. Lifestyle Modifications: Staying hydrated, avoiding irritants (like caffeine and alcohol), and practicing pelvic floor exercises may help improve urinary symptoms.
4. Monitor Symptoms: Keep track of your symptoms, including any changes in urination frequency, urgency, or discomfort. This information will be valuable for your healthcare provider.
Conclusion
While your symptoms may raise concerns about prostate health, they could also be related to other factors, such as phimosis or pelvic floor issues. A thorough evaluation by a healthcare professional is essential to determine the underlying cause and appropriate treatment. Remember, early intervention can often lead to better outcomes, so don’t hesitate to seek medical advice.
