Medication Consultation
Hello, doctor.
Could you please explain the effective uses of the antibiotics Pelvicillin 750 mg and Norfloxacin 625 mg for different symptoms? Thank you.
Xiao Xiansheng, 50~59 year old female. Ask Date: 2023/04/07
Dr. Huang Yiwen reply Pulmonology
Dear Mr.
Hsiao: The indications for Pefloxacin 750 mg enteric-coated tablets are community-acquired pneumonia and complicated urethritis, compared to Norfloxacin.
Additionally, Pefloxacin has fewer side effects or drug allergies.
Norfloxacin is also suitable for mild pneumonia and oral infections.
However, all medications must be prescribed by a physician; please do not self-medicate.
Changhua Hospital cares about you.

More Info
Pefloxacin and Norfloxacin are both fluoroquinolone antibiotics that are used to treat a variety of bacterial infections. While they share some similarities in their mechanism of action and spectrum of activity, they are typically used for different types of infections based on their pharmacokinetic properties and clinical indications.
Pefloxacin is often used for treating infections caused by Gram-negative bacteria, particularly in cases of complicated urinary tract infections (UTIs) and respiratory tract infections. It is effective against a range of pathogens, including Escherichia coli, Klebsiella pneumoniae, and some strains of Pseudomonas aeruginosa. Pefloxacin is also used in the treatment of skin and soft tissue infections, as well as bone and joint infections. Its broad-spectrum activity makes it a suitable choice for treating polymicrobial infections, where multiple types of bacteria may be involved.
Norfloxacin, on the other hand, is primarily indicated for uncomplicated urinary tract infections and is particularly effective against Gram-negative bacteria. It is often prescribed for cystitis and other lower urinary tract infections. Norfloxacin is less effective against Gram-positive bacteria and has a narrower spectrum of activity compared to Pefloxacin. It is also used for certain gastrointestinal infections, such as traveler’s diarrhea caused by susceptible strains of bacteria.
When considering the effective uses of these antibiotics, it is essential to take into account the specific infection being treated, the susceptibility of the bacteria involved, and the patient's overall health status. For instance, if a patient presents with a complicated UTI caused by a resistant strain of E. coli, Pefloxacin may be the preferred choice due to its broader spectrum of activity. Conversely, for a patient with an uncomplicated UTI, Norfloxacin may be sufficient and more appropriate.
Both antibiotics can have side effects, including gastrointestinal disturbances, central nervous system effects (such as dizziness or headache), and potential interactions with other medications. It is crucial to monitor patients for any adverse reactions and to ensure that the chosen antibiotic is appropriate for the specific infection being treated.
In terms of resistance, both Pefloxacin and Norfloxacin can be subject to bacterial resistance, which can limit their effectiveness. It is important for healthcare providers to consider local resistance patterns when prescribing these antibiotics. In cases where resistance is suspected or confirmed, alternative antibiotics may need to be considered.
In summary, Pefloxacin and Norfloxacin are valuable tools in the treatment of bacterial infections, but their use should be guided by the type of infection, the susceptibility of the bacteria, and the patient's clinical condition. Always consult with a healthcare professional for the most appropriate treatment options based on individual circumstances.
